Adult Long Term Care Unit Supervisor
Sawyer County Health and Human Services is accepting applications for a full-time Adult Long Term Care (LTC) Supervisor. Under the direct supervision of the Health and Human Services Director, the Adult Long Term Care Unit Supervisor provides management and oversight of budgets and grants relative to the Aging and Disability Resource Center-North, Sawyer County Branch, Adult Protective Services. The Adult LTC Unit Supervisor provides support and supervision to unit staff, including the Disability Benefit Specialist, Information and Assistance Specialists, and Adult Protective Services Specialists.
Sawyer County (population 18,000) is situated within the pristine woods and waters of northwestern Wisconsin.
Qualifications for this position include a minimum of a bachelor’s degree from an accredited social work program, human services program, or related field, and 2 years of relevant experience. Supervisory experience, including staff development and training.

Title: Adult Long Term Care Unit Supervisor
Department: Sawyer County Health and Human Services
The Aging and Disability Resource Center of the North/Sawyer County Branch
Reports to: Sawyer County Health and Human Services Director
Position Purpose:
The Adult Long Term Care (ALTC) Unit Supervisor serves as the Aging and Disability Resource Center of the North Sawyer County Branch Manager and Adult Protective Services (APS) Supervisor. The ALTC Supervisor provides leadership, supervision, and support to unit staff and oversees unit budget administration. The ALTC supervisor directly reports to the Health and Human Services Director and collaborates with the ADRC-N Regional Coordinator.
As with all positions within the agency, it is the duty of the individual in this position to fully support the mission, vision, and values of the agency, to work in a coordinated manner with other employees, and to demonstrate professional and personal confidence in the service delivery model and approaches taken by the organization.
The ALTC Supervisor is expected to integrate trauma-informed care into service to Sawyer County communities.
Essential Duties and Responsibilities

ADRC-N, Sawyer County Branch (ADRC-N/Sawyer) Manager
·
Support the ADRC-N/Sawyer to provide free, unbiased information related to aging or living with a disability, help people maintain their independence by connecting them with the right resources, and serve older adults and people with disabilities, as well as the families, friends, and caregivers who work with and care for them.
·
Supervise and support unit staff: Dementia Care Specialist (DCS), Elderly Benefit Specialist (EBS), Information and Referral Specialists (I&A), Regional I and A Specialist; and coordinate with ADRC-N/Price supervisor re: shared Disability Benefit Specialist (DBS) position as needed.
·
Complete all county and ADRC/ADRC-N supervisor training as well as ADRC Specialist training as needed.
·
Assure staff receive initial and ongoing training to perform essential services and assure activities are being appropriately reported to various databases.
·
Assure compliance with county and program (DBS, DCS, EBS, ADRC) policies.
·
Identify and report program/county-related issues to directors and participate in discussions/plans to address concerns.
·
Provide basic information and assistance services, options counseling, enrollment, and disenrollment counseling to customers of the ADRC-N/Sawyer as needed.
·
Manage the local ADRC-N budget, including completion of daily, monthly, and annual reporting to draw down federal Medical Assistance match allocations.
·
Conduct quality assurance and necessary adjustments relative to ADRC core functions by reviewing time and case noting in the state’s ADRC client tracking database and participate in state compliance reviews.
·
Coordinate outreach events to raise awareness of the ADRC-N/Sawyer County Branch and its functions.
·
Schedule and write agendas for ALTC Unit meetings.
·
Participate in coalitions related to ADRC target groups.
·
Attend meetings as scheduled, including HHS Administration meetings, HHS Board meetings, ADRC-N Branch Manager meetings, ADRC-N Management Team meetings, and ADRC-N Governing Board Meetings.
·
Maintain memberships as needed and attend statewide meetings relevant to the ADRC, i.e. ADPAW (Aging and Disability Professionals Association of Wisconsin), ADLIN (Aging, Disability, and Independent Living Network).
·
Administer and oversee Supplemental Security Income – Exceptional Expense (SSI-E) assessment and application by providing intake and assessment for eligibility determination. Review each non-family care SSI-E recipient annually for continued eligibility, need, and residency tracking requirements.
·
Oversee staff assignments in an equitable, balanced way to ensure timelines are met.
·
Review and approve staff 100%-time reporting for payroll and accurate data entry.
·
Serve as liaison to other groups and agencies in the ADRC-N/Sawyer County Branch region and develop positive professional relationships with service providers and community partners.
APS Supervisor:
·
Provide social services to elders and adults at risk, including information, investigation, and working in cooperation with other agencies, including law enforcement, in the areas of neglect or self-neglect, physical abuse, sexual abuse, emotional/psychological abuse, abandonment, or financial abuse.
·
Assure compliance with county and APS policies and best practices.
·
Provide leadership, supervision, and support for APS Specialist staff.
·
Coordinate and maintain weekly communications with APS Lead Specialist/APS Team.
·
Coordinate/Oversee intakes from the ADRC, law enforcement, citizens, banks, and other sources to the clerical APS intake worker(s).
·
Take APS intakes as needed.
·
Oversee APS client database tracking additions done by clerical APS intake worker(s).
·
Assign cases to the APS team. The supervisor will maintain a caseload as needed.
·
Support unit to remain in compliance with WI State Statutes Chapters 46.90, 51, 54, and 55.
·
Manage Adult-at-Risk and Elder Abuse/Neglect allocations via application for annual funds, track expenditures, request additional funds and assure required inner agency, HHS Board and Wisconsin Incident Tracking System state data reporting is completed.
·
Organize Sawyer County Adult Protective Services Multidisciplinary Team (aka I-Team) with four or more team meetings and/or subcommittee meetings per year.
·
Attend statewide and regional APS meetings, etc.
·
Coordinate with Sawyer County Probate and Corporation Counsel to provide oversight and management of all needed APS court action and protective placement court orders, including corresponding annual court review timelines.
·
Review and approve APS Specialist’s time study/time sheets.
·
Maintain positive professional relationships with LCO, Hayward Town, Hayward City Police, and the Sawyer County Sheriff’s Department.
·
Maintain positive professional relationships with the Senior Resource Center, the Waldo Center, LCO ADRS, Family Care, and IRIS organizations, and other community partners.
·
Provide outreach and education to the general public, ancillary agencies, community partners, and law enforcement.
·
Update forms and policies as needed.
Qualifications:

Education and Experience:
Bachelor's degree in social work or related human services field and at least 2 years of experience working with elders and or vulnerable adults. Supervisory experience, including staff development and training. Understanding of Wisconsin Statutes, Chapters 46, 51, 54, and 55. Possess knowledge regarding the service delivery system, the needs of the recipient group to be served, and the resources available or needing to be developed. Knowledge of personal computers, computer software applications, and data input experience, case assessment, planning, and management skills, and knowledge of long-term care resources. Knowledge of ADRC preferred. Valid motor vehicle operator’s license and proof of insurance.
