Southland Healthcare and Rehab is currently seeking compassionate, dependable, and motivated Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our nursing team for full-time 12-hour shifts. Requirements: Essential Job Duties and Responsibilities
  • Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies
  • Administer medications and treatments accurately and timely
  • Monitor residents for changes in condition and report concerns appropriately
  • Assess residents and communicate relevant findings to physicians, families, and the interdisciplinary team
  • Supervise and support Certified Nursing Assistants and other assigned staff
  • Ensure proper documentation of care, treatments, incidents, and resident status
  • Respond promptly and professionally to resident needs, emergencies, and call lights as needed
  • Review, implement, and update resident care plans as appropriate
  • Maintain infection control and safety standards throughout the facility
  • Participate in admissions, discharges, transfers, and other resident care coordination activities
  • Provide resident and family education when needed
  • Work collaboratively with team members to promote positive clinical outcomes and resident satisfaction
  • Ensure nursing services are delivered with dignity, compassion, and respect
Requirements
  • Current and active LPN or RN license in the State of [Insert State]
  • Current CPR certification preferred
  • Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred, but not required
  • Strong clinical assessment and critical thinking skills
  • Ability to supervise staff and work effectively as part of a team
  • Excellent communication and documentation skills
  • Dependable attendance and professional conduct
  • Ability to work 12-hour shifts and meet the physical demands of the position
  • Must pass required background screening and any additional pre-employment requirements in accordance with facility policy and state regulations

  • Experience with medication administration and treatment documentation
  • Knowledge of long-term care regulations and standards
  • Strong organizational and time-management skills
  • Compassionate, resident-centered approach to care
  • Ability to remain calm and effective in a fast-paced environment
