About This Position
Southland Healthcare and Rehab is currently seeking compassionate, dependable, and motivated Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our nursing team for full-time 12-hour shifts. Requirements: Essential Job Duties and Responsibilities- Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies
- Administer medications and treatments accurately and timely
- Monitor residents for changes in condition and report concerns appropriately
- Assess residents and communicate relevant findings to physicians, families, and the interdisciplinary team
- Supervise and support Certified Nursing Assistants and other assigned staff
- Ensure proper documentation of care, treatments, incidents, and resident status
- Respond promptly and professionally to resident needs, emergencies, and call lights as needed
- Review, implement, and update resident care plans as appropriate
- Maintain infection control and safety standards throughout the facility
- Participate in admissions, discharges, transfers, and other resident care coordination activities
- Provide resident and family education when needed
- Work collaboratively with team members to promote positive clinical outcomes and resident satisfaction
- Ensure nursing services are delivered with dignity, compassion, and respect
- Current and active LPN or RN license in the State of [Insert State]
- Current CPR certification preferred
- Previous experience in long-term care, skilled nursing, rehabilitation, or post-acute care preferred, but not required
- Strong clinical assessment and critical thinking skills
- Ability to supervise staff and work effectively as part of a team
- Excellent communication and documentation skills
- Dependable attendance and professional conduct
- Ability to work 12-hour shifts and meet the physical demands of the position
- Must pass required background screening and any additional pre-employment requirements in accordance with facility policy and state regulations
- Experience with medication administration and treatment documentation
- Knowledge of long-term care regulations and standards
- Strong organizational and time-management skills
