Sylva Valley Health & Rehab is hiring Licensed Practical Nurses (LPNs) to join a strong in-house team focused on teamwork, reliable coverage, and quality resident care.

? Self-Scheduling Pilot: We’re rolling out a self-scheduling pilot to improve work-life balance and reduce last-minute schedule changes.
? RN Pathway: If you would like to become a Registered Nurse (RN), we want to support your goals. Training/schooling support may be available for eligible team members (details discussed during interview).

• 12-hour shifts: 7a–7p / 7p–7a

• Provide direct nursing care per physician orders and care plans
• Administer medications and treatments safely and accurately
• Observe, document, and report changes in condition appropriately
• Collaborate with CNAs, nurses, providers, and the interdisciplinary team
• Maintain clear, timely documentation and follow facility policies

• Active Licensed Practical Nurse (LPN) license (state-licensed)
• Skilled nursing/long-term care experience preferred
• Strong clinical skills, teamwork, and dependable attendance
