Homecare Registered Nurse at UK St. Claire – Morehead, Kentucky
About This Position
The Registered Nurse collects and analyses patient data, formulates an individualized plan of care, determines progress toward the attainment of outcomes, coordinates care delivery, promotes a safe practice environment, and advocates mutual respect with patients, families, and other healthcare providers.
Duties/Responsibilities:
- Conducts in-home assessments, and develops, and evaluates the plan of care in partnership with the patient, representative, and caregiver(s).
- Communicates significant clinical changes or findings to the physicians involved in the plan of care and other health care practitioners.
- Educates patients and families on disease management, medication administration, and preventative care.
- Monitors vital signs and observes patients for any changes in health condition and provides wound care.
- Collaborates with other healthcare professionals to ensure holistic care for patients.
- Maintains accurate and up-to-date medical records and documentation timely.
- Supervises and instructs home health aides. Accepts student nurses as directed.
- Takes on-call during evening hours, nights, holidays, and weekends on a rotating basis.
Education:
Associate Degree in Nursing (ADN) required. Bachelor Degree Nursing (BSN) preferred.
Experience:
Two years’ experience preferred.
Licensure/Certification:
Valid Kentucky Board of Nursing (KBN) license required. Valid Kentucky driver's license required.
