SCHEDULE
Monday – Friday, 8:30AM – 4:30PM
SUMMARY
Family Service League is seeking a full-time Case Manager for the Health Home program in Bay Shore. The Case Manager provides comprehensive care coordination and support to individuals with complex behavioral health, medical, and/or substance use needs. This role ensures that all program visit and documentation requirements set forth by the New York State Department of Health (DOH) for the Health Home program are met, while promoting client engagement, wellness, and recovery through person-centered services.

QUALIFICATIONS
Bachelor’s Degree in a health-related field is required. Master’s Degree preferred.
Minimum of 2 years of experience providing direct services to people with serious mental illness, medical conditions, developmental disabilities, and/or substance abuse preferred.
Proficiency in Microsoft Office required; experience with an EHR is preferred.
Strong assessment, engagement, organizational and time management skills.
Excellent verbal and written communication skills are required.
Valid and clean New York State Driver’s License required.
PHYSICAL REQUIREMENTS
This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
