We’re seeking a dedicated and empathetic Social Services Coordinator to support the emotional and social well-being of our residents. This role is ideal for someone who thrives on building relationships, guiding life transitions, and creating programs that foster connection and quality of life.
What You’ll Do:

• Provide one-on-one emotional support and counseling to residents and families
• Assess and document residents’ social and emotional wellness
• Coordinate care plans and collaborate with internal teams and external providers
• Lead signature programs like New Resident Orientation and Continuum of Care Education
• Monitor residents for signs of isolation or decline and coordinate interventions
• Educate residents and staff on wellness, safety, and resident rights
• Maintain accurate documentation and uphold HIPAA standards

Requirements: What You Bring:

• Bachelor’s degree in social work, counseling, or related field
• Social Work license or eligibility to obtain one within 6 months
• Experience in senior living, CCRC, or similar setting preferred
• Strong communication, empathy, and organizational skills
• Proficiency in Microsoft Office and basic computer systems
