About This Position
Join the CPLC LUCES team (Latinos Unidos Contra el Sida)! We are hiring a bilingual Medical Case Manager to bring much needed case management support to the community. CPLC LUCES provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.
POSITION SUMMARY
Under general supervision, the case manager provides direct covered services to clients living with an HIV diagnosis, coordinates with community support and stakeholders. The case manager is responsible for managing appointment schedules, assigned caseloads as a clinical liaison to medical services, conducting needs assessments, implementing treatment plans, facilitating referrals, and participating in ongoing learning experiences. They provide services in both Spanish and English, helping clients navigate systems and barriers to access.
Case management includes providing coordinating access to primary medical care, dental care, medication adherence, nutrition, home health care, legal services, transportation services, mental health care, substance abuse services, psychosocial support services, and other necessary resources.
MINIMUM QUALIFICATIONS AND COMPETENCIES
- Bachelor’s degree from an accredited college in a licensed* field related to case management such as social work, nursing, public health or other human services related field preferred.
- Or a high school diploma and comparable professional knowledge, skills and abilities that document 4 years of experience specific to medical case management (conducting needs assessments, implementing treatment plans, facilitating referrals) may be substituted for the degree.
- Bilingual Spanish/English is required to communicate with Spanish-speaking clients, considering the importance of knowing & communicating medical terminology in both languages.
- Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STDs.
- Must successfully complete the agency’s orientation and demonstrate competency, knowledge, and skills.
- Have good organization and time management skills.
This position requires the ability to work a flexible schedule as needed per program requirements, including driving out into the community to meet clients where they are.
