Position Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our team. The RN will provide case management services and oversee RN-delegated tasks to support caregivers in the community. This role offers the opportunity to coach, mentor, and lead individuals in their professional development while ensuring high-quality care standards.

Key Responsibilities:
  • Coordinates and manages the care solutions needs of clients by providing training for caregivers in accordance with Community Living Alliance (CLA) Policies, state regulations and rules.
  • Conduct assessment visits with clients in accordance with CLA policies and State Regulations and Rules.
  • Evaluates the need for services and makes referrals to other services as appropriate.
  • Develops a person centered plan of care for the client, giving full consideration to the client’s preferences for service arrangements and choice of caregivers. Secures written orders from the client’s physician, if applicable to Care Solutions.
  • Provides administrative, coordination and direct service support in the Care Solutions team for clients.
  • Monitoring and approves time and cares documentation provided by CLA caregivers and Supported Living Partners.
  • Audits CLA Supported Living (SL) and CLA Medical Assistance files to ensure compliance with Medicaid regulations.
  • Validate caregivers time and service through electronic visit verification system (EVV).
  • Supervise, mentor and develop caregivers on RN’s caseload upon hire and throughout duration of providing care.

Minimum Education/Experience and Requirements:
Required
  • Must be 18 years old or older
  • Must be authorized to work in the United States
  • Associate's degree in nursing
  • Current licensure as a Registered Nurse under s. 441.06, Wisconsin Statues
  • Valid driver's license, reliable transportation, and current auto insurance to travel throughout Dane County
  • Must be able to push, pull, and/or lift up to 35 pounds
  • Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.
  • Satisfactory Results of a WI Caregiver Background Check

Preferred
  • Bachelor's in nursing
  • 1-year supervisory experience in personal care services, community-based home care (non-skilled nursing) or related program
  • 3 years nursing experience in personal care services, or related program
  • Experience working with people with disabilities and elderly
