About This Position

RiseWell Community Services is looking for a full-time Registered Nurses to join our Forensic Assertive Community Treatment (FACT) Team in Richmond Hill, Queens NY.

This individual will provide management and administration of medications, in conjunction with the team Psychiatrist, and assess the health needs of the clients. The Registered Nurse will serve as a member of a mobile, community-based, multidisciplinary treatment team for persons with severed and persistent mental illness, delivering individualized care to those in need.

QUALIFICATIONS:

  • Current licensure as a registered professional nurse by the New York State Education Department
  • Minimum 1 year of experience in a treatment or rehabilitation setting
  • A valid and clean NY State driver license
  • Fingerprinting, criminal record check
  • Approval from NYS Office of Mental Health
  • Strong documentation and organizational skills
  • Effective time management skills
  • Knowledge of nursing assessment methods
  • Skilled in counseling techniques through education, training, or work experience
  • Ability to work as a generalist

CORE & ESSENTIAL RESPONSIBILITY:

  • Conduct physical health assessments on clients
  • Assess physical health needs of clients
  • Make appropriate referrals to community physicians
  • Provide management and administration of medications in conjunction with the team psychiatrist
  • Participate on a team that delivers an integrated array of treatment, rehabilitation, and support services customized to each client
  • Provide the majority of services in the community or where most convenient for client
  • Utilize all agency based IT applications pertaining to this position in an efficient manner
  • Remain current on changing developments in individual field of expertise
