Duties/Responsibilities:
- Performs home health care to patients. Administers prescribed medications and treatments in accordance with nursing standards.
- Reviews and maintains patients’ records, charts, and other pertinent information. Post tests and examination results.
- Observes, records, and reports patient’s condition and reaction to drugs and treatments to physicians. Dispenses medication as directed.
- Prepares equipment and aids physicians during treatment, examination, and testing of patients.
- Maintains cooperative relationships among the home health care team by communicating information, responding to requests, building rapport, and participating in team problem-solving methods.
- Maintains patient confidentiality.
- Ensures quality and safe delivery of home health care.
- Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
- Supports and enforces infection-control policies and procedures.
- Performs related work as required.
Required Skills/Abilities:
- Comprehensive knowledge of all phases of home health care.
- Has administrative and clinical skills.
- Is sensitive to the needs and concerns of patients and the home health care team.
- Excellent communication and writing skills.
- Has the ability to supervise and train staff.
- Ability to take and give directions.
- Ability to carry out treatment as ordered and document appropriately within the realm of practice
- Has the ability to use mature nursing judgment in daily decision-making.
- Knowledge of medical terminology and medication.
Education and Experience:
- Valid state Physical Therapist license
- Bachelor’s degree preferably in a science-related field
- Excellent work ethic and dedication to patient success
- Basic computer skills
- CPR and first aid certification required
- A minimum of two years of experience working in the physical therapy field is preferred.
Physical Requirements:
- Frequent exposure to communicable diseases and other conditions common in a clinic environment.
- Prolonged periods of sitting and working at a computer.
- Occasionally you may lift and carry boxes of material and equipment weighing 10-15 pounds.
