Job Summary
The Inboxologist Physician Assistant or Nurse Practitioner provides comprehensive patient care and clinical support at Trillium Health. This role is responsible for managing EPIC in-basket communications—including lab results, refills, and patient messages—while also performing same-day patient visits and providing provider coverage during absences.
This position serves across both primary and specialty care areas, including HIV/Hepatitis C, PrEP/PEP, LGBTQ+ health, MOUD, and sexual health services. The Inboxologist role requires strong clinical judgment, collaboration, and adaptability to support continuity of care and an exceptional patient experience.
Duties & ResponsibilitiesPatient Care (Approx. 50%)
- Deliver care across Trillium Health’s primary and specialty services, including HIV, PrEP, PEP, sexual health, Hepatitis C, LGBTQ+ care, and opioid use disorder treatment.
- Provide provider coverage for call-outs, LOAs, and PTO.
- Advise patients on nutrition, stress, sexuality, contraception, smoking cessation, and substance use.
- Collaborate with physicians to manage nursing interventions.
- Complete peer and chart reviews.
- Pursue ongoing continuing education.
- Manage EPIC in-basket items including lab results, refills, and patient messages.
- Coordinate with providers and clinical teams to ensure seamless care.
- Assess patient concerns, triage appropriately, and escalate as needed.
- Process refills and update prescriptions; consult with pharmacists when necessary.
- Follow organizational policies, best practices, and clinical guidelines.
Required Skills & Abilities
- Minimum 2 years of primary care experience as a board‑certified NP or PA.
- Strong interpersonal, teamwork, and patient‑communication skills.
- Proficiency with electronic medical records.
- Excellent written, verbal, and computer skills.
- Ability to respect and support individuals with diverse lived experiences.

Education & Experience
Nurse Practitioners:
- M.S. degree from an NLN‑accredited nursing school.
- Current NYS NP licensure and national certification (ANA or equivalent).
- Must maintain a valid practice agreement OR meet criteria for 3,600 independent hours per NYS law.
Physician Assistants:
- Graduate of an ARC‑accredited PA program.
- Bachelor’s degree (or higher) in physician assistant studies from a NYS‑recognized program.
- Must submit a practice agreement to NYSED within 90 days of hire.
All APPs:
- Unrestricted DEA registration and BLS certification required.
- Experience in community‑based healthcare preferred.

Physical Requirements:
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 50 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
