About This Position
POSITION SUMMARY
This is a part-time (24 hours per week) position that assists the Managing Director of Congregate Care and coordinates with associated providers, managers, clinicians, and other involved staff to manage youth health care. The Nurse focuses on providing care and support to youth with developmental disabilities and complex care needs who are receiving outpatient treatment. The Nurse works collaboratively with a multidisciplinary team to implement individualized treatment plans, promote mental health education, and ensure high-quality client care.
The Nurse assists in ensuring that youth receive all required routine and specialized medical care, including daily medication administration and management of medication administration records (MAR). Responsibilities include transporting youth to medical appointments, tracking appointment schedules, and maintaining required documentation.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Schedules medical appointments for routine and specialized care; ensures required insurance and Medical cards accompany the youth to the appointment; provides or coordinates transportation as needed; and communicates examination outcomes to the Managing Director of Congregate Care and program staff.
- Responsible for recording and checking the medical information for each child in the files.
- Ensure that all forms of medication are disbursed to the respective homes.
- Ensure each youth has current physical, dental, eye and hearing examinations and all immunizations required by state law.
- Responsible for ensuring payment requests are submitted for physicians and securing of medical cards.
- Assign who will respond to colds, fevers, and routine health care in each setting.
- Ensures employees and residents receive required Hepatitis B vaccinations and Tuberculosis (TB) testing in accordance with agency policy and applicable regulations.
- Ensure that all people within the milieu team secure necessary written approval before medical procedures are executed and psychotropic medication is dispensed.
- Provide schedule of clients needing to have psychotropic medication evaluated with the consulting psychiatrist.
- Trains staff about Bloodborne Pathogens, Hepatitis B vaccinations requirements and medication administration policies.
- Ensure that all required information is entered into agency systems in a timely manner and that all required services are completed.
- Adhere to Agency personnel policies.
- Enter information on daily medication logs and home logs regarding events of the day.
- Practices in compliance with the Illinois Nurse Practice Act and all applicable licensure requirements.
- Completes reports for child health files and maintains comprehensive tracking report of upcoming medical appointments.
- Ensure all transitional living homes maintain appropriate First Aid kits and required medical supplies.
- Responsible for billing for on-site medical visits and daily medication administration.
- Scheduling Hazmat pickup.
- Provides basic first aid as required in accordance with training and scope of practice.
- Any additional duties delegated by the Managing Director of Congregate Care.
QUALIFICATION REQUIREMENTS
- Ability to learn and implement the CARE practice model and apply CARE principles in daily programming.
- Ability to complete required Therapeutic Crisis Intervention (TCI) training, successfully pass written and physical components, and maintain required refresher certifications.
- Demonstrated ability to engage youth in a respectful, supportive, and developmentally appropriate manner.
- Effective communication, interpersonal, and organizational skills.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Commitment to person-centered care, continuous improvement, and professional standards.
- Must meet DCFS/CANTS Clearance Requirements.
- Must be at least 21 years of age.
- Must possess and maintain a valid driver’s license and maintain proof of automobile insurance.
EDUCATION AND/OR EXPERIENCE
- Current, active Illinois Licensed Practical Nurse (LPN) License required. Preferred: Registered Nurse (RN) license.
- Two years experience in public health nursing, community nursing, or maternal-child health nursing preferred.
- Experience working with children with special needs strongly preferred.
- Demonstrated ability to train staff on basic healthcare skills, first aid, and recognition of medical needs.
- Ability to work effectively with children requiring specialized care in a supportive and professional manner.

Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire. Ongoing application of these principles is expected and essential to successful job performance.
TCI (Therapeutic Crisis Intervention). Employees must:
- Pass the written test annually
- Pass the physical test every six months (if applicable to their role)

WORKING CONDITIONS
This position operates in a professional office, clinical, and residential environment and may require travel to agency sites, client locations, and partner organizations based on role responsibilities.
The role involves periods of walking, standing, or sitting, and may occasionally require climbing stairs, bending, kneeling, or lifting materials up to 25 pounds, depending on job duties.
Minimal exposure to environmental factors such as noise, dust, or varying indoor/outdoor temperatures may occur depending on program needs and client-related activities
