About This Position

Job Summary:

The Patient Outcomes Coordinator is accountable for providing leadership in planning, implementing, evaluating, and integrating performance measurement and improvement activities. Facilitates the improvement of patient care processes and outcomes using evidence-based approaches with the input and clinical experience of those who will be providing the care. Performs data analysis for quality measures with responsibility for performance improvement. Responsible for patient outcome information management functions including design, analysis, and interpretation. Collaborates with administrative and clinical leaders to resolve outcome issues. Demonstrates knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs. Performs other duties assigned by the QM Director.

Performance Expectations:

Performance expectations will be covered in the orientation to your job.

    Demonstrates ability to review and abstract pertinent clinical information from the electronic medical record. Demonstrates ability to analyze data and identify opportunities for improvement. Demonstrates ability to effectively communicate with managers, staff and physicians. Demonstrates ability to accurately enter pertinent clinical information into third party vendor data collection tools.

Qualifications:

Education/Skills

Graduate of a recognized non-online RN program program required. Bachelor degree preferred with statistical and basic computer knowledge.

Work Experience:

A minimum of five years of diverse clinical nursing practice preferred.

Certification/Licensure-DUE UPON HIRE

  • Current License with the MS State Board of Nursing

Mental Demands:

Demonstrate and motivate others to have initiative, be creative, set priorities and deadlines, focus on performance, assure accuracy, be detail and outcome oriented, and function under stress. Excellent oral and written skills to effectively relate to hospital, medical staff, and external regulatory agencies.
