LCSW – CCHBC Program at RiseWell Community Services – Wyandanch, New York

RiseWell Community Services is seeking a Full Time LCSW to handle intakes to join the CCHBC Program. Our Clinical Services are designed to empower both adults and children/adolescents on their journey to recovery, RiseWell offers comprehensive outpatient mental health and addiction services to individuals in need. Our mission is to provide unwavering support and guidance to individuals seeking assistance through individual and group therapy, medication-assisted treatment, and comprehensive support services.

In this role, you will be responsible for screening and conducting intakes for individuals seeking admission to the Wyandanch Clinic. Conducts assessments to determine individual’s needs and preferences and develops recommendation for the person’s Initial Treatment Plan. Conducts new client intakes and coordinates with all referral sources and collaterals. Maintains appropriate records according to program regulations. Provide clinical evidence-based services with a focus on integrated mental health, physical health, and substance use recovery. Service delivery will occur on-site, in vivo, and in the community. A caseload of adults, children, and families will be maintained while providing individual, group, and family sessions.

QUALIFICATIONS:
Licensed Clinical Social Worker (LCSW). Experience working in an outpatient mental health clinic and conducting intakes with individuals with mental health diagnoses. A valid and clean NY State driver license, fingerprinting, criminal record check, and approval from NYS Office of Mental Health.
