About This Position
General Responsibilities:
- Provides intensive, strength based, one-on-one support to clients in collaboration with the clinician and reflective of the youth’s mental health treatment plan.
- Establishes relationships with youth, foster families, biological families, case managers, and other providers in order to deliver comprehensive services
- Interviews clients, performs observations, and helps patients build a plan to address behaviors.
- Customize behavioral and safety plans for individual clients.
- Provides interventions for foster parents and care provider staff.
- Coordinates care and behavioral planning support with foster parents.
- Provides feedback to clients, foster parents and care provider staff on behavior plan.
- Recognize and document critical improvements in client behaviors.
- Identifies skills, supports and resources that could be used to enhance youths’ wellness, including internal and in the community.
- Assists in the education and supervision of youth in group sessions and activities as needed.
- Coordinates with other service providers and assists clinical team in building network and resources.
- Maintains ongoing client documentation as required including progress notes, treatment planning, and assessments.
- Ability to follow HIPAA guidelines as well as mental health federal & state regulations.
- Transports and accompanies youth to appointments as assigned.
- Attends internal and external trainings and applies newly gained knowledge in providing services.
- Attends and prepares for required court hearings (if applicable).
Education/Training: Bachelor’s degree in Social Work, Psychology, or related field of study from an accredited college required. Master’s degree in Social Services or related field of study from an accredited college, preferred.
