Behavioral Support Crisis Intervention Specialist/Case Aid (Global Refuge) at Diakon Lutheran Social Ministries – York, Pennsylvania

• Provides intensive, strength based, one-on-one support to clients in collaboration with the clinician and reflective of the youth’s mental health treatment plan.
• Establishes relationships with youth, foster families, biological families, case managers, and other providers in order to deliver comprehensive services
• Interviews clients, performs observations, and helps patients build a plan to address behaviors.
• Customize behavioral and safety plans for individual clients.
• Provides interventions for foster parents and care provider staff.
• Coordinates care and behavioral planning support with foster parents.
• Provides feedback to clients, foster parents and care provider staff on behavior plan.
• Recognize and document critical improvements in client behaviors.
• Identifies skills, supports and resources that could be used to enhance youths’ wellness, including internal and in the community.
• Assists in the education and supervision of youth in group sessions and activities as needed.
• Coordinates with other service providers and assists clinical team in building network and resources.
• Maintains ongoing client documentation as required including progress notes, treatment planning, and assessments.
• Ability to follow HIPAA guidelines as well as mental health federal & state regulations.
• Transports and accompanies youth to appointments as assigned.
• Attends internal and external trainings and applies newly gained knowledge in providing services.
• Attends and prepares for required court hearings (if applicable).

Education/Training: Bachelor’s degree in Social Work, Psychology, or related field of study from an accredited college required. Master’s degree in Social Services or related field of study from an accredited college, preferred.
