Are you passionate about helping patients regain independence in the comfort of their homes? The VNA of Hanover & Spring Grove is seeking a dedicated Occupational Therapist to join our Home Health team. This role plays a vital part in delivering exceptional, patient-centered care and supporting our mission to provide high-quality home-based services with integrity, respect, teamwork, and excellence.

• Evaluate patients’ functional status and home environments

• Create individualized treatment plans focused on daily living skills and independence

• Educate patients and caregivers on assistive devices and energy conservation

• Collaborate closely with physicians, nurses, therapists, and caregivers

• Provide palliative therapy services for hospice patients as needed

• Ensure documentation and care meet Medicare and state regulations

• Graduate of an accredited Occupational Therapy program

• Licensed Occupational Therapist in Pennsylvania

• At least 2 years of OT experience (home health preferred)

• Valid driver’s license, reliable transportation, and insurance

• CPR certification and clean background check
