Physical Therapist (full-time) at Accommodate Restore & Collaborate LLC – LaGrange, Illinois

Accommodate, Restore & Collaborate, LLC is seeking a full-time Physical Therapist at Meadowbrook Nursing & Rehabilitation in LaGrange, IL. We are looking for a PT to prevent, assess, diagnose, and treat gait, mobility, and function in adult patients at this site. The job duties include, but are not limited to, the following:

• Performing evaluations/plans of care and providing treatment accordingly.
• Appropriately and effectively completing documentation.
• Screening residents and individuals who may benefit from therapy.
• Communicating patient status and needs to the patients, family, caregivers, or other members involved with patient care.
• Directing and supervising the work of assistants as applicable.
• Expanding and enhancing self, in both clinical and professional knowledge through associations and continuing education.
• Providing in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
• Supporting and participating in department operations/meetings.

• Licensed in the state of Illinois as a Physical Therapist
• Ability to communicate well with patients, families, supervisors, and colleagues.
• Comfortable with computers.
• Preferred experience working in a SNF, but we are open to other practice area experience
