Description

MAJOR DUTIES AND RESPONSIBILITIES
• Observe, report, and assess client self-administration of medication.
• Identify resources for clients to overcome barriers to care, such as transportation, housing, and childcare arrangements.
• Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance.
• Maintain strict confidentiality in accordance with agency policies.
• Engages direct working relationships with and serves as primary point of contact for clients, facilitating medical and program services.
• Visits clients' homes on a regular basis
• Accompany clients to medical appointments, when required.
• Deliver in-home weekly or monthly health education and promote HIV self-­ management to clients.
• Communicate with Care Team members (Care Coordinator, primary care physicians and other health care providers) to facilitate client care.
• May meet with clients after primary care physician appointments to review and update care plan with the Care Coordinator.
• Meet with Care Team (including, but not limited to, Care Coordinator and primary care provider) to discuss client care issues and needs and facilitate client health care.
• Maintain documentation of all client encounters and complete reporting requirements according to organization standards.
• Track client information, schedules, files, and forms in a confidential manner
• Track client attendance at medical appointments and patient navigation sessions and initiate outreach and missed appointment procedures, as necessary.
• Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor.
• Accurate and timely documentation of service rendered.
• Accurate and timely billing for services rendered.
• Daily completion of service tracking logs.
• Conduct Rapid HIV testing.
• Participate in Networking events.
• Provide weekend (Saturday) coverage.



Qualifications

EDUCATION AND CERTIFICATION AND EXPERIENCE
• Associate Degree
• At least three (2) years of experience working with underserved populations.
• Certified as peer worker by the NYC AIDS Institute Peer Certification and/or case management certification by CUCS and/or at least ninety (90) hours of documented
• Bilingual (English/Spanish) Preferred


CONFIDENTIALITY: Responsible for the confidentiality of all matters relating to patient care and financial activity. Any breach will lead to disciplinary action up to and including dismissal.

CORPORATE COMPLIANCE: As a condition of employment, the employee agrees to fully participate in, and cooperate with the organizations’ Corporate Compliance. If, at the sole discretion of Brooklyn Plaza Medical Center, employee fails to participate/cooperate fully with Corporate Compliance it will lead to disciplinary action up to and including termination.
HOURS OF WORK: Minimum 40 hours, alternating Saturdays.
