Job Title: General Manager of Care Navigation & Chronic Care Management

Location: Oakbrook Terrace, IL

Reports To: Vice President of Care Continuity & Partnerships

Type: Full-Time, Senior Leadership

Salary: $150,000 - $180,000

Variable Compensation: This role includes a significant performance-based incentive, offering the potential to earn up to 50% of your base salary annually.

Position Summary

The General Manager of Care Navigation and Chronic Care Management (CCM) is a high-impact scale stage operator role responsible for the design, implementation, and methodical scaling of our clinical platform. This individual will sit at the intersection of clinical and operational excellence ensuring that our care models not only improve patient outcomes but also serve as a primary engine for organizational growth.

The ideal candidate is a "clinical architect" who understands how to leverage technology, data, and human capital to manage high-risk populations across the continuum of care while meeting ambitious growth KPIs. This role will require a highly hands on, execution oriented leader who is comfortable operating at the ground level and owning day to day execution. The GM will be directly involved in day to day operations, workflow design, staff performance management, and problem-solving, particularly during periods of growth. This position is intentionally operational and build-focused, rather than a traditional high-level executive role centered primarily on strategy and oversight.

Core Responsibilities1. Strategic Growth & Market Expansion
  • Scale Strategy: Develop and execute a multi-year roadmap for the expansion of CCM and Care Navigation services into new geographic markets and payer segments (Medicare Advantage, ACOs, Commercial).
  • Product Innovation: Evolve traditional CCM into a "Care-as-a-Service" model, integrating Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) and other value added services to increase per-patient revenue.
  • Partnership Development: Collaborate with the Business Development team to articulate clinical capabilities to health systems and skilled nursing facilities, acting as the clinical subject matter expert.
2. Clinical Operations & Model Design
  • Workflow Optimization: Design evidence-based clinical protocols for chronic conditions (e.g., Diabetes, COPD, CHF, CKD) that maximize navigator efficiency without sacrificing quality.
  • Tech Stack Integration: Oversee the selection and optimization of Care Management Platforms, ensuring seamless integration with various EMRs and automated patient engagement tools.
  • Compliance & Quality: Ensure all programs meet or exceed CMS requirements for CCM (CPT 99490, 99487, etc.) and maintain NCQA or URAC accreditation standards.
3. Financial Performance & ROI
  • P&L Management: Full accountability for the Care Navigation department’s budget, focusing on "Cost to Collect" and "Clinical ROI."
  • Value-Based Care (VBC) Strategy: Align CCM activities with VBC contracts to capture shared savings through Gap Closure and Risk Adjustment Factor (RAF) accuracy.
  • Utilization Management: Track and report on key performance indicators (KPIs) such as Total Cost of Care (TCOC) reduction and Bed Day savings.
4. Leadership & Talent Development
  • Building the Bench: Recruit and lead a multidisciplinary team of RN Case Managers, and non-clinical Case Managers, Care Coordinators & Clinical Liaisons.
  • Culture of Performance: Establish a high-accountability culture driven by metrics like "Patient Touches per Day," "Enrollment Conversion Rates," and "Patient Retention."
Required Experience & Qualifications
  • Education: Master’s degree in Healthcare Administration (MHA), Business (MBA), or Nursing (MSN). A clinical background (RN, NP, PA) is highly preferred.
  • Experience: 10+ years in clinical operations leadership, with at least 5 years specifically focused on Chronic Care Management or Population Health.
  • Track Record: Proven success in scaling a clinical service line (e.g., grew a patient census from 1,000 to 10,000+). 5+ years of experience managing large teams (50+) and scaling them.
  • Regulatory Knowledge: Deep expertise in CMS billing codes, Medicare Advantage Star Ratings, and HIPAA compliance.
