Summary of Job Description:

The Youth Behavioral Health Case Manager works in partnership with the client and the Behavioral Health care team to provide responsive, culturally sensitive, and comprehensive services. The Case Manager partners with clients aged 13-25.
Focus on individuals who identify as BIPOC, Latinx, and LGBTQ+. Youth facing systemic barriers, including: Housing insecurity or unstable living situations, disconnection from school, education, or employment, family rejection or lack of support, engagement in survival behaviors (e.g., navigating unsafe environments, informal work), involvement with the juvenile justice system or other legal challenges. Youth who benefit from trauma-informed, culturally responsive programs designed to support resilience, safety, and growth, and backgrounds, persistent mental health and substance use disorder concerns, and complex needs. The goal of the Case Manager is to improve clients’ stability, wellness, recovery, and social determinants of health. The Case Manager assists with referrals and coordination of care. Works with multiple systems, including but not limited to schools, jails, hospitals, vocational rehabilitation and housing programs, criminal justice systems, and medical providers.

Key Responsibilities

• Provide culturally responsive advocacy, outreach, information, and referral services to clients in need of case management, including wraparound supports that address behavioral health, housing, food security, education, employment, and safety.
• Coordinate services with the Behavioral Health care team, internal programs, and external service providers—including medical, legal, housing, and social service agencies—to ensure integrated and holistic care that reflects the client’s cultural values and lived experiences.
• Monitor and support the implementation of individualized crisis plans, ensuring they are culturally and linguistically appropriate and responsive to the client’s environment and support system.
• Provide timely crisis intervention and stabilization services, including de-escalation and safety planning, with sensitivity to trauma and cultural context.
• Partner with the system navigator to help clients access essential community resources such as medical care, housing assistance, food programs, transportation, childcare, and legal advocacy.
• Conduct community-based education, outreach, and engagement activities to reduce stigma, increase awareness of behavioral health services, and build trust within Latino communities.
• Provide transportation for behavioral health clients to therapy groups and other services as needed, using Consejo vehicles, while ensuring safety and respect for client dignity.
• Offer supportive services to family members and other individuals identified as important to the client, recognizing their role in the client’s emotional well-being and recovery.
• Advocate on behalf of clients to promote their continued engagement in school, home, and community life--with a focus on youth development, family reunification, and community integration.
• Attend mandatory clinical trainings, supervision, and regularly scheduled department and agency meetings to support professional growth and program quality.
• Demonstrate a strong understanding of and ability to provide culturally and linguistically appropriate services, with a commitment to equity and inclusion for Latino individuals and families.
• Complete all required documentation accurately and promptly to ensure compliance with agency, state, and federal standards.
• Serve on committees, task forces, and special assignments as directed to support agency initiatives and community partnerships.
• Perform other duties as assigned to support the mission and operations of the agency.

Qualifications

• Bachelor's degree or AA with 2 or more years of experience in a field related to social, behavioral, or human services, or equivalent volunteer experience comparable to two years of full-time work in a related setting.
• Experience working with a mental health population preferred.
• Obtain the proper credentialing through the state of Washington as an Agency Affiliated Counselor
• Safely operate a motor vehicle and have a valid driver’s license.
• Proficient in speaking, reading, and writing both English and Spanish.
• Proficient in Microsoft Office products
• Lifting requirements up to 25 lbs.

WORK SCHEDULE
This full-time in-person position works a typical schedule of 8 AM - 5 PM, Monday to Friday.

SALARY RANGE

$ 27.00 to $ 32.45 an hour, depending on experience, qualifications, and credentials.
