Park Place Seniors Living is seeking a Care Manager to lead, support, and inspire our nursing team while ensuring high-quality, individualized care for our residents.
This role is ideal for an experienced RN or LPN who thrives in a collaborative environment and wants to make a real impact on residents, families, and staff.
About the RoleThe Care Manager provides clinical and operational leadership to the nursing care team, ensuring safe, compassionate, and evidence‑based care aligned with residents’ needs, choices, and care plans. Working closely with the Site Leader and Director of Care, you will play a key role in quality improvement, staff development, and fostering a positive and respectful workplace culture.
You will also be a trusted point of contact for residents, families, physicians, and health authority partners.
What You’ll DoLeadership & Team Development- Recruit, orient, coach, and lead nursing team members to deliver consistent, high-quality resident care
- Foster a respectful, collaborative, and accountable team culture
- Provide mentorship, performance feedback, and ongoing education
- Conduct performance appraisals and address performance concerns when required
- Ensure comprehensive resident assessments are completed on admission and ongoing (e.g., MDS/InterRAI)
- Lead individualized care planning based on resident needs, preferences, and best practices
- Support nursing staff in advocating for resident choice, dignity, and autonomy
- Build strong therapeutic relationships with residents and families
- Champion Park Place’s culture of safety and resident well-being
- Ensure documentation meets professional, regulatory, and organizational standards
- Monitor staffing levels to support quality care and safe operations
- Participate in and lead quality improvement initiatives, accreditation standards, and site committees (e.g., Nursing Practice Council, Pharmacy & Therapeutics)
- Act as a key liaison with families, physicians, and health authority representatives
- Communicate clearly, professionally, and compassionately with all stakeholders
- Support conflict resolution and effective problem-solving within the care team
- Promote and model safe work practices and occupational health and safety compliance
- Educate staff annually on workplace violence, bullying, and harassment prevention
- Investigate and report incidents, near misses, and hazards
- Ensure follow-up and completion of safety action plans
- Lead by example in all aspects of workplace safety
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current practicing registration
- Baccalaureate of Nursing with post-basic Gerontology, or
Three (3) years of recent, related experience, or an equivalent combination of education and experience
- Strong clinical knowledge in gerontology and evidence‑based practice
- Proven leadership, coaching, and team‑building skills
- Excellent communication and interpersonal abilities
- Sound judgment, critical thinking, and problem-solving skills
- Ability to manage change, priorities, and competing demands
- Commitment to professionalism, resident dignity, and continuous improvement
- Proficiency with clinical documentation systems and basic computer applications
