ADRC-N Dementia Care Specialist/Information and Assistance Specialist

Sawyer County Health and Human Services is accepting applications for a full-time Aging Disability Resource Center of the North (ADRC-N) Dementia Care Specialist/Information and Assistance Specialist. Under the direct supervision of the Adult Long Term Supervisor, the ADRC-N Dementia Care Specialist/Information and Assistance Specialist, through the Aging and Disability Resource Center of the North (ADRC-N), provides information and assistance about Alzheimer’s Disease and other dementias to individuals, family caregivers, ADRC staff, and the community at large. The specialist is expected to raise awareness about the unique needs of individuals with dementia by working with family caregivers, assisting county programs to be better prepared with knowledge and resources, and acting as a catalyst to create dementia friendly communities. Moreover, the specialist delivers
information about services, programs, resources (public and private) to meet the long term care needs of adult Sawyer County residents. As such, the specialist can help maximize opportunities for self-sufficiency and choice for target groups, including adults with physical and/or developmental disabilities, and elders. The ADRC-N Dementia Care Specialist/Information and Assistance Specialist is expected to integrate trauma informed care while serving our communities.

Qualifications for this position:
Graduate of an accredited college or university with a bachelor’s degree in a health or human services-related field or Registered Nursing degree. Must have at least one year of experience working with individuals, such as older adults (aged 60 years and older) and/or individuals living with developmental/intellectual disabilities, physical disabilities, mental health or substance abuse disorders, who constitute one of the target populations of the Aging and Disability Resource Center.
• Must become memory screen certified within 30 days of hire.
• Must obtain the online State Functional Screen Training Course within 30 days of hire.
• Valid driver’s license and unlimited access to reliable transportation required.
Competitive wage and benefit package includes health insurance, Wisconsin Retirement System, life insurance, sick and vacation leave, and holiday pay.
Starting salary $56,950 - $60,195 DOQ.

Hours:
Fulltime (40 hours per week)

Position Purpose:

Under the direct supervision of the Adult Long Term Supervisor, the ADRC-N Dementia Care Specialist/Information and Assistance Specialist, through the Aging and Disability Resource Center of the North (ADRC-N),
provides information and assistance about Alzheimer’s Disease and other dementias to individuals, family caregivers, ADRC staff, and the community at large. The specialist is expected to raise awareness about the unique needs of individuals with dementia by working with family caregivers, assisting county programs to be better prepared with knowledge and resources, and acting as a catalyst to create dementia friendly communities. Moreover, the specialist delivers

information about services, programs, and resources (public and private) to meet the long-term care needs of adult Sawyer County residents. As such, the specialist can help maximize opportunities for self-sufficiency and choice for target groups, including adults with physical and/or developmental disabilities, and elders.
The ADRC-N Dementia Care Specialist/Information and Assistance Specialist is expected to integrate trauma-informed care into their work serving our communities.

Essential Duties and Responsibilities:

Dementia Care Specialist
:

·
Develop referral relationships with physicians, dementia diagnostic clinics, and other health providers.

·
Provide dementia-specific consultation and technical assistance related to cognitive screening and individual and family caregiver issues to adult protective services (APS) agencies, crisis response systems, county and tribal aging offices, aging and disability resource center (ADRC) staff members, and other county agencies, as requested.

·
Provide at least two approved evidence-based or evidence-informed interventions, one specifically designed for family caregivers, as part of ongoing activities. Maintain fidelity to the intervention model and accommodate families’ schedules for meetings (e.g., evenings and weekends as needed).

·
Collaborate actively and develop referral protocols with local, regional, and statewide dementia organizations, including Wisconsin Alzheimer’s Institute diagnostic clinics, the Alzheimer’s Association chapters, and the Alzheimer’s and Dementia Alliance of Wisconsin.

·
Complete training for and administer Memory Screens

·
Offer and provide person-centered support when memory screens are positive to assure opportunities for follow-up with a primary care provider are available in accordance with the individual’s preferences.

·
Offer and provide dementia care consultations for individuals with dementia and/or their caregivers. Coordinate with other ADRCs, county and tribal aging units, APS, and other county staff members and be available for joint or independent home visits.

·
Serve as a catalyst for developing and implementing strategies to create and sustain dementia-friendly communities in the ADRC service area.

·
Provide outreach and awareness to professionals, employers, organizations, and the general community about the ADRC and available dementia services.

·
Maintain current knowledge of dementia, research findings, new evidence-based interventions, and other innovations for people living with dementia and their caregivers.

·
Participate actively in DCS statewide and regional meetings and training programs. Assist colleagues by sharing experiences. Mentor new DCS staff and provide materials and products to other ADRCs, as requested.

·
Provide conference workshops as requested and appropriate to increase the awareness of dementia and available services.

·
Complete 100% time reporting, collect and report program data, and contribute to DCS program evaluation.

Information and Assistance Specialist
:

·
Provide consumer-driven information and assistance on a wide variety of topics that are accurate, objective, and relevant to the individual’s expressed need and presented in language and formats that are easy for consumers to understand

·
Gather sufficient information to accurately identify and clarify the inquirer’s issues and needs.

·
Determine the range of needs through motivational interviews, home visits, collateral contacts, etc.

·
Provides information about public and/or private programs, services and benefits capable of meeting the consumer’s needs and actively assists the consumer in accessing resources or making referrals.

·
Responds to initial inquiries and requests for information and assistance within 24 hours or by the end of the next business day and schedules appointments within 10 business days following the consumer’s request.

·
Provides options counseling to consumers, their families, and caregivers about the options available to meet long-term care needs and factors to consider in making long-term care decisions.

·
Administers Long-Term Care Functional Screen to determine an individual’s functional eligibility for managed long-term care and IRIS.

·
Provides information and counseling to assist consumers in the process of enrollment and disenrollment from managed care or IRIS.

·
Provide ADRC customers with assistance in connecting with the appropriate resources and services to meet their needs.

·
Provide follow-up with customers to determine if needs were met or if additional assistance is necessary.

·
Prepare SSI-E certifications.

·
Participate in section and department meetings, work-related committees, and trainings.

·
Answer ADRC Regional phones as needed.

Ability to prepare and maintain a variety of documents, including client service plans and recommendations, program evaluations, client assessments, treatment plans, and therapy notes.

Ability to use tact and courtesy in maintaining an effective working relationship with department employees, county supervisors, county officials, and the general public.
