Job Description

Suncoast Center, a leader in quality behavioral health services, is seeking a community liaison to join our mental health team. This role is essential in providing screenings and case management to adults within the community. The position requires strong organizational skills, professionalism, and the ability to work in a fast-paced, client-centered environment.

Job Duties:

  • Meets with clients to accurately and rapidly assess and document the client’s symptoms, diagnosis, and interventions, and assures that all program criteria are met for opening the case.
  • Completes all documentation to open the client into targeted case management (TCM)
  • Completes an accurate and up to date assessments, functional assessments and service plan within 30 days of the client’s case being opened, and every six months thereafter.
  • Assists clients in developing realistic, effective and measurable care plans. Establishes prioritized behaviorally descriptive, realistic desired changes with clients, and develops a realistic plan for their accomplishment with appropriate time frames.
  • Acts as an Advocate for the client at all relevant staff meetings, court hearings, etc., or arranges for a substitute.
  • Uses appropriate crisis intervention skills and anticipates, and prevents, crisis where possible.
  • Provides case management services to clients in the Adult Case Management program
  • Provides services in a way that utilizes the principles of recovery, and treats every client as a partner in a process to better their lives.
  • Remains current in the general knowledge of psychotropic medication, side effects, and DSM V diagnoses, and recognizes when medical services are appropriate.
  • Completes all progress notes and other documentation (i.e. time sheets, outcome measures, and service worksheets).
  • Meets productivity standard.
  • Attend monthly treatment team meetings on a virtual platform for clients that have been civilly committed to the Florida state hospitals.
  • Responds to public placement referrals at local crisis stabilization units in a timely manner and screens clients for public placement.
  • Reviews the case with LCSW/LMHC/LMFT and makes appropriate recommendations (i.e. referrals and placement in community or public placement).
  • Completes all paperwork to be submitted at Baker Act hearings, and attends all hearings to advocate for public or community placement.
  • Completes all individualized service plans for identified clients
  • Accomplishes individual goals
  • Provides direct clinical services that use solution-oriented strategies of intervention and principles of recovery that are consistent with best practice principles, uses respectful and competency-based words and actions with clients and families while ensuring that client oriented treatment plan goals are incorporated that include symptom management by medication and therapy and promote personal growth and development, vocational plans and substance use, if applicable.
  • Provides ongoing support to the Manager when completing required weekly, monthly and quarterly reports.
  • Provide other duties as assigned.

Minimum Qualifications:

Education: A bachelor's degree from an accredited college or university in any field.

Experience: 1 year work experience providing at least 12 months of related case management services to either the adult or child population.

Additional requirements:

  • Completion of AHCA approved targeted case management training within 3 months of hire.
  • Must be certified through the Florida Certification Board (FCB) within 6 months of hire.
  • Must be able to complete a level II background screening and pre-employment drug test in compliance with Florida Law. Must be at least 21 year of age. Must be able to provide official transcripts.
