- Patient Support:
- A. Will receive the patients from Patient Development department through a warm handoff process.
- B. Attend the patients doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers.
- C. Conduct home visits to assess patients needs, deliver resources, and provide additional assistance as required.
- D. Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding.
2. Resource Delivery:
- A. Deliver comprehensive patient binders to patients homes containing plan of care and training information.
3. Communication:
- A Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care.
- B. Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team.
- C. Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care.
3. Communication:
- A Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care.
- B Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team.
- C Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care.
- Strong work ethic and relationship building skills.
- Self-motivated with the ability to manage time appropriately.
- Strong communication skills inside of the office and outside.
- Experience with MS Office products (Outlook, Word, Excel, etc.)
- Minimum associate level education preferred. (High school diploma or equivalent is required.)
- Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
- Must have valid driver's license and your own transportation.
- Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices.
- Healthcare experience preferred.
