• Patient Support:
• A. Will receive the patients from Patient Development department through a warm handoff process.
• B. Attend the patients doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers.
• C. Conduct home visits to assess patients needs, deliver resources, and provide additional assistance as required.
• D. Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding.

2. Resource Delivery:

• A. Deliver comprehensive patient binders to patients homes containing plan of care and training information.

3. Communication:

• A Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care.
• B. Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team.
• C. Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care.

3. Communication:

• A Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care.

Minimum Qualifications:

• Strong work ethic and relationship building skills.
• Self-motivated with the ability to manage time appropriately.
• Strong communication skills inside of the office and outside.
• Experience with MS Office products (Outlook, Word, Excel, etc.)
• Minimum associate level education preferred. (High school diploma or equivalent is required.)
• Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
• Must have valid driver's license and your own transportation.
• Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices.
• Healthcare experience preferred.

$24 - $26 an hour
