Home Care Licensed Healthcare Professional
We are seeking a dedicated and compassionate Licensed Healthcare Professional to join our team, providing essential medical oversight and monitoring for our In-Home Support Services (IHSS) and Home and Community Based Services (HCBS) programs. Key Responsibilities:
- Conduct comprehensive client assessments and develop individualized care plans in collaboration with the Home Care Supervisor and Case Management Agency.
- Perform annual and 90-day in-person medical visits for clients and attendants to monitor health status and ensure compliance with care protocols.
- Monitor changes in client health and update care plans accordingly to reflect evolving needs.
- Conduct in-home client evaluations to assess the effectiveness of care plans and provide support to the Case Management Agency as needed.
- Develop and maintain accurate, detailed client records, including assessments, care plans, and documentation of changes or updates.
- Collaborate with clients, families, and care teams to ensure coordinated and effective care delivery.
- Stay informed about best practices and regulatory requirements related to home healthcare services.
Requirements:Skills and Qualifications:
- Valid and current license as a Healthcare Professional (e.g., Registered Nurse, Licensed Practical Nurse, or equivalent).
- Proven experience in home healthcare, client assessment, and care planning.
- Strong interpersonal and communication skills to effectively interact with clients, families, and healthcare teams.
- Ability to perform in-home evaluations and adapt care plans based on individual client needs.
- Excellent organizational skills with attention to detail in documentation and record-keeping.
- Knowledge of relevant healthcare regulations and compliance standards.
- Ability to work independently and manage a flexible schedule.
