About This Position
Job Summary: Provides patient centered navigation services and care coordination across the continuum of care . Functions as the primary point of contact for the cancer patient: from the time of their cancer diagnosis and throughout the treatment continuum. Responsible for assessing, developing, implementing and evaluating effectiveness and quality of navigation services. Provides community outreach activities. Reports data and implements quality improvement initiatives. Acts a liaison among patients, families, health care providers, staff and community to provide a seamless transition throughout phases of care.
Qualifications:Bachelor’s degreefrom a recognized college or university required. Master’s degree preferred. Current RN license to practice in NC (NC license or multi-state compact license). At least three years of acute care nursing or oncology nursing experience required. Navigation certification or Oncology Certified Nurse certification preferred. Broad knowledge of community health concepts, services and resources. Must have strong organization skills with initiative, creativity, and public speaking skills. Requires ability to work in a self-directed environment with the ability to network, organize and coordinate a patient care team. BLS required. Certification/Advanced Disease Education as required by Oncology Service Line Leadership (Once testing requirements are met; testing/certification must be completed within 12 months). "Experience with Breast Cancer preferred".
