Position: LMHC Location: Mt. Dora and LecantoSchedule: Monday-FridayEmployment Type: Full-TimeOpen to part time 1099 to cover Lecanto only Job SummaryWe are seeking a Licensed Mental Health Counselor (LMHC) or Psychologist to provide services within post-acute care facilities in the Miami area. The provider will travel to multiple facilities, conduct patient rounds, and collaborate closely with psychiatric division providers and interdisciplinary healthcare teams.The ideal candidate is compassionate, self-motivated, and experienced in working within post-acute or skilled nursing environments.Must hold an active Florida license.

Primary ResponsibilitiesConduct patient evaluations and obtain detailed medical and psychological historiesPerform accurate assessments, diagnosis, treatment, consultation, education, and follow-up careMake routine rounds in post-acute care facilitiesCommunicate effectively with patients, facility staff, physicians, and other healthcare professionalsDevelop and submit comprehensive health care plans for periodic physician reviewPromote health and wellness through patient education (diet, hygiene, disease prevention)Refer patients to specialists as appropriateReport deaths and contagious diseases to governmental authorities as requiredMaintain proper and compliant charting protocolsPerform other duties as assignedEducation & Experience RequirementsActive and unrestricted State of Florida license (LMHC or Psychologist)Current National Board CertificationExperience in post-acute, skilled nursing, or long-term care settings preferredKnowledge, Skills & AbilitiesStrong written, verbal, and electronic communication skillsAbility to work independently and travel between facilitiesDemonstrated initiative, creativity, and problem-solving abilityCollaborative mindset with interdisciplinary healthcare teamsProficiency in electronic medical records (EMR) systemsBilingual (English/Spanish) preferred but not requiredReporting StructureReports to the Supervisory Psychiatric Physician / Executive Director.Physical RequirementsAbility to communicate effectively with patients and staffAbility to sit, stand, or use a computer for extended periodsAbility to perform repetitive hand and wrist motionsAbility to use hands and fingers to handle or feel objectsAbility to reach with hands and armsIf you're a motivated Florida-licensed mental health provider seeking flexibility with growth potential, this is an excellent opportunity to join a collaborative psychiatric team serving post-acute care patients.
