About This Position
Upstate Family Health Care Inc. is a Federally Qualified Health Center (FQHC) offering family health care services to individuals of all ages. Hours of operation are Monday - Friday 8:00am - 6:00 pm

We are currently accepting applications for a Contract Primary Care RN.
This is a 1099 Contract Position.
$50-60 per hour, $4000-4800 for 2 weeks for 80 hours; Additional incentives available
Schedule: 4-day work week, Monday - Friday
Job Summary:
The RN participates in the multidisciplinary process of providing care and treatment to patients, and provides clinical support and technical assistance to the professional, medical and other staff. The RN is responsible for providing health services using a Patient Centered Medical Home (PCMH) team-based care model to individuals through effective partnerships with patients, their caregivers/families, and community resources in their primary care physician. The RN is a Clinical Lead who’s Primary responsibilities include but are not limited to monitoring their clinical team workflow, delegating tasks as needed to help with daily clinical team flow, completes paperwork to the fullest within the RN scope of practice to handover to the provider to review and sign, and tending to patient sick calls. The Team model consists of 1 MD, 3 NP, 2 LPN, and 1 Medical Asst.
Other Daily Team Model Clinical Activities to Monitor and assist with when needed:
Room patients which includes vitals (Ht, Wt, BP, Temp, Pulse, Resp, O2 Sat) and any required questionnaires.
Perform Point of Care Testing as ordered by the Provider.
Administer office medications as prescribed by Providers.
Maintains exam rooms for necessary supplies and materials.
Prepares equipment and assists providers during treatment, examination and testing of patients.
Re-enforces patient/family education about diagnostic procedures, nutrition and maintenance of health and wellness at the request of the Provider.
Prepares patients for physician examination.
Screens patients for appropriate information. Re-enforces Instructions to patients in collection of samples and tests.
Maintains/reviews patient records, charts and other pertinent information.
Maintains patient confidentiality.
Attending in-service training, team and other appropriate medical, staff and health center meetings as delegated by the Director of Clinical Operations.
Universal Duties:
Attends any mandated continuing education/training programs.
Comply with all pertinent regulatory and agency regulations and operating standards.
Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency’s culture, values, and mission.
Participate in activities as part of the team that may include meetings, trainings, and committees.
Promote and maintain a safe working environment.
Other duties as assigned.
Required Experience:
1 to 2 years experience
Unencumbered Registered Nursing License
1-3 years’ experience in medical office preferred
1-3 years of experience working with EMR systems and data reporting.
Intermediate PC skills (Excel, PowerPoint, Word, Outlook, Share Point)
Ability to act calmly and effectively in an emergency situation.
Ability to be flexible to respond to changing demands.
Exceptional communication and interpersonal skills.
Physical Demands:
The physical demands described here are representative of those that be met by and employee successfully perform the essential functions of this job and include the ability to type and operate a personal computer with various software programs, effectively operate standard office equipment, bend, stoop, crouch, kneel, twist, balance, and work at a desk, lift and carry up to 60 pounds, and communicate (written and oral) in a clear and professional manner.
Establish and maintain effective work relationships with co-workers and patients, maintain regular attendance, understand and carry out a variety of oral and written instructions, have knowledge of proper English usage, grammar, punctuation, spelling, and vocabulary, have the ability to learn office principles, practices, and methods, understand filing systems, including numerical, alphabetical, and chronological, learn a variety of procedures, policies, and services of the assigned work unit or program, perform assigned duties with efficiency and accuracy and maintain confidentiality.
Certain deadlines and unanticipated developments may require work during evenings and weekends.
Ability to quickly address any emergent issues without losing focus on the task at hand.
