What You’ll DoSupport Clients & Caregivers
Conduct home visits, check-ins, and client intakes to ensure each person receives compassionate, high-quality care. Be the friendly, reliable point of contact that helps clients and Direct Care Workers feel supported.Coordinate & Communicate
Keep everyone in the loop — supervisors, caregivers, clients, and case managers — so that care is smooth, timely, and personalized.Organize & Track
Help manage schedules, documentation, and basic data entry. Keep client and caregiver information complete and accurate, so that the team can focus on what matters most: people.Step In Where Needed
Occasionally, provide in-home support when a caregiver is unavailable. Help out with trainings, events, and program activities as needed.
Heart and Humility: You are motivated by serving others, especially those who may be vulnerable, and you show up ready to listen, help, and support.
Energetic & Nimble: You can juggle multiple priorities, adapt to changing needs, and respond quickly.
Bilingual Skills: English/Spanish fluency is required to connect with clients and caregivers.
Comfortable in the Office & in the Field: You can handle basic scheduling, data entry, and administrative tasks, and also step into homes to observe, support, and engage.
Team-Oriented: You communicate clearly, collaborate well, and share information in a positive, constructive way.
Experience in healthcare, community support, nonprofit operations, or any role where you’ve supported people or coordinated services is a plus — but your heart for service matters most.
RequirementsMust have or be able to obtain a valid Arizona Fingerprint Clearance Card (Level I).
Must pass central registry background screening.
Valid Arizona driver’s license and reliable transportation required.
High School Diploma or GED
6+ months working with elderly adults or people with disabilities (preferred)
Entry-level Excel or comfort with basic spreadsheets
Direct Care Worker training provided upon hire.
