Requirements:

The Director of Care and Transitions is responsible for planning, organizing, developing, and directing the social services programs of the Skilled Nursing Facility in accordance with federal, state, and local standards, as well as facility policies and procedures. The Director ensures that each resident’s social, emotional, and psychosocial needs are identified and met, while facilitating smooth admissions and discharges that promote continuity of care and optimal quality of life.

Key Responsibilities

  • Develop and implement social services policies and procedures that comply with CMS regulations and facility standards.
  • Conduct comprehensive psychosocial assessments for each resident upon admission, quarterly, annually, and as needed for significant changes in condition.
  • Participate in interdisciplinary care plan meetings to develop and update individualized care plans.
  • Coordinate and manage all aspects of the discharge planning process, including assessments, family communication, education, and arranging post-discharge services such as home health, hospice, outpatient rehabilitation, or community programs.
  • Ensure safe, timely, and well-documented discharge of residents/patients, including completion of discharge summaries and communication with external providers.
  • Assist residents and families in adjusting to the facility environment and transitions of care, including admissions, transfers, and discharges.
  • Provide counseling and support to residents and families dealing with emotional, financial, or adjustment issues.
  • Advocate for resident rights and ensure dignity and respect are maintained at all times.
  • Maintain accurate, thorough, and confidential documentation in accordance with HIPAA and facility policy.
  • Develop relationships with community agencies, hospitals, and other healthcare organizations to coordinate discharge and continuity of care.
  • Supervise and train social services staff and assist in staff education on psychosocial needs and discharge procedures.
  • Participate in quality assurance activities and surveys to ensure compliance with regulatory standards.

Job Type: Full-time

Pay:$60,000.00 per year
