FFS Provider - Wyandanch Clinic at RiseWell Community Services – Wyandanch, New York

RiseWell Community Services is seeking a Fee For Service Provider to join the Wyandanch Clinic. Our Clinical Services are designed to empower both adults and children/adolescents on their journey to recovery, RiseWell offers comprehensive outpatient mental health and addiction services to individuals in need. Our mission is to provide unwavering support and guidance to individuals seeking assistance through individual and group therapy, medication-assisted treatment, and comprehensive support services.

Provide clinical evidence-based therapy in individual, family, and group modalities to a caseload of children and adults with substance use diagnoses. Work as part of an interdisciplinary team. Complete all required documentation within a 48 hour period and comply with program regulations.

QUALIFICATIONS:
Valid NYS LMSW, LMHC, or LCSW with minimum 3 years of experience in a treatment or rehabilitation setting with a minimum 1 year focus on community based crisis services. Valid and clean NY State driver license, fingerprinting, criminal record check, and approval from NYS required.
