About This Position
The ICS Lead Worker provides direct support, coaching, and skills training to adults receiving Integrated Community Supports (ICS) services in a provider-controlled apartment setting. The Lead Worker also supports daily service coordination, documentation compliance, staff support, and quality assurance activities to ensure services align with person-centered goals, individual service plans, and Minnesota DHS requirements for ICS and 245D services.
Essential Duties and Responsibilities
A. Direct Support & Skill Building (Primary Function)
1. Deliver person-centered supports and training to help individuals maintain stability and independence in their home and community.
2. Provide support in areas such as:
o Daily living skills, routines, and household management
o Community participation and engagement
o Health, safety, wellness, and risk awareness
o Interpersonal communication, self-advocacy, and coping strategies
3. Promote participant choice, dignity, independence, and culturally responsive service delivery.
4. Support participants in accessing community resources (transportation training, employment supports, benefits navigation, appointments, etc.), as applicable to the service plan.

B. ICS Service Coordination Support (Lead Worker Functions)
1. Serve as the on-site lead for shift structure and service flow, ensuring appropriate coverage, prioritization, and responsiveness.
2. Assist in reviewing service delivery expectations with assigned staff and ensuring services match authorized plans.
3. Support implementation of individualized strategies, positive support approaches, and de-escalation methods as applicable.
4. Communicate urgent service issues to the supervisor/designated coordinator promptly (safety concerns, incidents, unmet needs, suspected maltreatment, etc.).

C. Documentation, Compliance & Billing Support
1. Complete accurate and timely documentation for services delivered, including:
o Daily notes / service notes
o Goal progress documentation
o Incident documentation when applicable
2. Ensure documentation is completed according to internal standards and required timelines.
3. Maintain compliance with DHS enrollment/service requirements applicable to ICS services and provider operations.

D. Health, Safety & Participant Protection
1. Maintain a safe, respectful, and trauma-informed environment for all participants.
2. Follow emergency procedures, safety plans, and risk protocols.
3. Report suspected abuse, neglect, or exploitation immediately according to policy and law.
4. Assist participants with medication reminders/observations only as authorized and trained.

E. Staff Support & Training Assistance (Non-Supervisory or Delegated)
1. Support onboarding of new staff through shadowing, modeling best practices, and coaching.
2. Provide real-time coaching to DSP/support staff on:
o Person-centered service delivery
o Documentation quality
o Professional boundaries and ethics
o Cultural responsiveness
3. Participate in team meetings, training, and quality improvement activities.

F. Professional Conduct & Communication
1. Maintain confidentiality and comply with HIPAA/data privacy expectations.
2. Communicate respectfully with participants, guardians (when applicable), case managers, and interdisciplinary partners.
3. Maintain professional boundaries and avoid conflicts of interest.

Minimum Qualifications
1. High school diploma or GED required (Associate degree or higher preferred in Human Services, Social Work, Psychology, or related field).
2. At least 2 years of experience providing direct support or related HCBS services preferred.
3. Ability to work independently, prioritize tasks, and respond appropriately to urgent needs.
4. Ability to complete required documentation accurately and on time.
5. Valid driver’s license and proof of insurance (if driving is required for the position).

Required Compliance Qualifications (Minnesota DHS / HCBS)
1. Must be able to pass a DHS background study and maintain eligibility for direct-contact services.
2. Ability to work in a 245D/HCBS-regulated environment, following applicable policies and procedures.
3. Ability to complete and maintain required trainings (timelines per agency policy and applicable standards), such as:
o Orientation and program training
o Person-centered practices
o Emergency procedures
o Participant rights and protections
o Vulnerable adult reporting
o De-escalation / positive supports (as assigned)

Preferred Qualifications
• Experience working in provider-controlled or supported housing programs
• Experience supporting adults with disabilities, mental health diagnoses, brain injury, or complex care needs
• Strong coaching/mentoring skills with peer staff
• Familiarity with person-centered service planning and waiver services

Knowledge, Skills, and Abilities
• Working knowledge of HCBS ethics, boundaries, and person-centered practice
• Strong verbal and written communication skills
• Ability to remain calm under pressure and manage conflict professionally
• Time management and organization skills
• Ability to use electronic documentation systems and basic office technology

Physical & Work Environment Requirements
• Ability to stand, walk, bend, and assist with light physical support as needed during shift activities
• Ability to lift up to [25–50] pounds, as required by program needs
• Ability to respond to emergency situations in a timely manner
• Work occurs primarily in participant apartment settings and common areas; may include community-based activities
