Our GA Community Action Treatment (CAT) program is recruiting for a Service Coordinator. Program is located in region 1 and 2 (near Cherokee county and surrounding areas).

DESCRIPTION:
Georgia Community Action Treatment (CAT) is intended to be a safe and effective alternative for children with serious behavioral health conditions that are creating crisis in the family. Upon successful completion, the family should have the skills and support system needed to maintain improvements made during services. The goals of the CAT program are to:
- Strengthen the family and support systems for youth and your adults to assist them to live successfully in the community.
- Decrease out – of – home placements, including psychiatric hospitalizations and Department of Juvenile Justice (DJJ) involvement.
- Improve family and youth functioning.
- Decrease substance use and abuse.
- Transition into age - appropriate services; and
- Increase health and wellness.

RESPONSIBILITES:
- Collect, update, and organize family-specific data related to cases from assessment to discharge.
- Audit, upload, and coordinate with staff regarding their documentation, assessments, and other required documentation.
- Adhere to the fidelity of the Community Action Treatment service standard.
- Maintain minutes for staff, supervision meetings and additional meetings as they occur.
- Assist team members within the program with identifying and connecting with local resources in each region served.
- Coordinate and schedule meetings with referral sources and community supports as needed
- Assist staff with family services as requested. (Safety checks, transportation, etc.)
- Conduct ongoing file audits.
- Manage and ensure staff complete weekly documentation as outlined within the scope of work for the program.
- Report to work as scheduled.
- Provide in person and remote training to new program staff.

QUALIFICATIONS:
Minimum Qualifications:
- In a graduate program pursuing a master’s degree in a field related to applied behavior analysis, social work, or clinical counseling;
- At least two years’ experience with individuals with mental health and or substance abuse diagnosis
- Trained in crisis de-escalation methodology;
- Valid CPR and First Aid Certification;
- Valid Driver’s License and auto insurance;
- Must be computer literate;
- Must be experienced and competent in profession and maintain any applicable training, license and/or certification
Preferred Qualifications:
- Four (4) years’ experience working with individuals with individuals with mental health and or substance abuse diagnosis of which at least 2 years included case manager experience
- Working knowledge of the Georgia Department of Children and Family Services system
