Job Description:

- Dispense medications and provide pharmaceutical care to patients in a Federally Qualified Health Center (FQHC) setting.

- Collaborate with healthcare providers to ensure safe and effective medication use for patients.

- Conduct medication therapy management and medication reconciliation services.

- Educate patients on proper medication use, potential side effects, and drug interactions.

- Maintain accurate and up-to-date patient medication records.

- Participate in quality improvement initiatives related to medication management.
