About This Position
2nd Shift: 40 Hrs
Sunday - Thursday 3pm - 11pm
Rate $27.85
Scope of Service: Position serves as the supplemental supervisor while on shift in the absence of the program manager. Acts in a professional and respectful manner responding to: staffing questions and issues which arise on the shift, client concerns, keeping staff accountable for assigned tasks, scheduling, etc. Position is responsible to participate in the On-Call rotation and supervise shift staff as assigned.
Direct Care responsibilities include: supporting individuals to learn and enhance activities of daily living by providing hands-on assistance when necessary; utilizing positive behavioral interventions strategies, ensuring medical appointments occur as scheduled, and fostering involvement in social and community activities while maintaining safety of the individual. Performs other duties as assigned.
ESSENTIAL JOB FUNCTIONS:
Individual Care and Programmatic Responsibilities
Is responsible as the shift lead to supervise, support, and maintain positive working relationship with staff and individuals.
Supports program manager with scheduling, on-call responsibilities, continuity of care for the program, and other assigned program specific administrative duties.
• rapport, working relationship, and demonstrates sensitivity and commitment to individuals.
• Provides face-to-face hands-on supportive assistance enabling individuals to maintain living in their apartment/home, including activities of daily living, personal care, and other activities which promote healthy living.
• Ensure staff encourage the use of natural supports, and fosters involvement in social and community activities while maintaining individual safety.
• Ensure individuals attend routine medical, psychological, dental appointments, personal business, and other appointments according to program schedule/Therap including transport. May be required to use own vehicle for appointment transport in emergency situations, if agency vehicle is unavailable.
• Provides input in Individual Plan development supporting individuals to achieve aspirations and goals.
• Attends and participates in identified collaborative team meetings.
• Works with community providers for quality care of individuals.
• Maintains individual records and reporting documentation as indicated by the agency and through DDS regulations.
• Responsible for daily Therap data entry and review of ISP’s, interval data and time tracking. Will follow-up with staff who have missed entries to ensure completion.
• Responsible to follow state and agency guidelines to administer medication and review Kardex to ensure agency and state guidelines are being followed.
• Attends and participates in monthly staff meetings.
• Maintains all appropriate trainings required by the agency and whenever possible attends agency sponsored in-services, outside trainings, conferences and meetings to facilitate continued professional growth.
Environmental Responsibilities
• Participates in agency vehicle upkeep and maintenance monthly.
• Maintains a clean, well-organized work environment.
• Responsible for ensuring the up keep and sanitation of common areas, kitchens, bathrooms and so forth as identified and applicable.
• Participates in property upkeep and general office maintenance that supports workplace safety.
• Completes other designated environmental task as assigned.
Quality Assurance – Chart Documentation and Professional Development
Adheres to policies and practices as outlined in the personnel policy manual, individual program manuals, and as outlined by the quality assurance and safety committees.
• Demonstrates competency for population served.
• Maintains individual records as assigned and in accordance with contractual, licensing, and accreditation requirements.
• Participates in agency committee work when available or as permitted.
• Attends and participates in agency meetings, team meetings, and inter-disciplinary team meetings upon request.
• Conducts self in a professional manner at meetings, on the job site, and during integration with individuals in the community in accordance with agency policies.
QUALIFICATIONS
• Bachelor’s Degree preferred with a minimum of one-year experience in a related field working with individuals with intellectual disabilities and/or autism. High School diploma considered with a minimum of five-years’ experience.
• Valid Connecticut Driver’s license with clean driving record is required.
• Completion of certifications of CPR & First Aid, PMT, Medication Certification, and all other trainings as required by DDS & Continuum of Care.
• Computer skills required including working knowledge of Microsoft office.
• Knowledge of the community support system and resources is preferred.
• Available to work evenings, weekends, and overnights.
