About This Position

Description:

The Supportive Housing Services (SHS) Case Manager provides supportive services at a Permanent Supportive Housing complex, The Nel. The SHS case manager will work in collaboration with community providers and housing providers for people with disabilities to obtain and maintain permanent supportive housing.

The schedule for this position is Wednesday - Saturday 8am 6:30pm.

Requirements:

Education & Experience

  • Lived experience of homelessness, mental health, and/or substance use and a combination of 2 years of related employment, education, or volunteer experience
    OR
  • BA/BS in mental health, social services, or a related field plus one year of experience supporting individuals with mental illness, housing instability, and/or substance use
    OR
  • Eligible for certification as a Qualified Mental Health Associate (QMHA) through MHACBO within one year of hire

Licensing & Background Requirements

  • Valid Oregon Driver License (per organizational vehicle use policy)
  • Successful completion of required background check, fingerprinting, and urinalysis drug screen, including a negative THC result prior to employment

Knowledge, Skills, & Abilities

  • Ability to communicate effectively with individuals experiencing severe mental health symptoms and/or substance use challenges
  • Experience working with diverse populations and multiple community agencies
  • Ability to provide professional, supportive services using trauma-informed and culturally responsive approaches
  • Ability to engage participants in person and by phone and manage a varied, fast-paced workload
  • Proficiency with mobile technology, computers, electronic records systems, and standard office equipment
  • Ability to multitask and complete documentation within required timelines
  • Ability to work both independently and collaboratively within a team
  • Knowledge of community resources and ability to work within HIPAA and confidentiality requirements
  • Ability to meet the physical demands of the role, including lifting up to 25 lbs., standing, sitting, bending, kneeling, crouching, reaching, grasping, tolerating strong odors, repetitive motion, extensive computer use, and local travel
