Job Overview:

RWJBarnabas Health is seeking a highly dedicated Oncology Care Transition Coordinator for our Morris Cancer Center at the New Brunswick campus.

The Oncology Care Transition Coordinator is a registered professional nurse who under the supervision of the Oncology Care Transitions Manager contributes to the care of any hospitalized cancer patients by engaging patients in the transitional care management process and by collaborating with the inpatient and ambulatory clinicians.

This person meets with patients upon admission to the inpatient unit, participates in multidisciplinary daily rounds, ensures the coordination and confirmation of appointments, follow up requirements, and needs for discharge.

Qualifications:

Required:

• BSN – bachelor’s in science in Nursing
• Graduate of an accredited school of nursing

Preferred:

• At least 3-5 years of Oncology RN experience
• Oncology or BMTU Certification preferred
• OCN Certification preferred

Certifications and Licenses Required:

• BLS and ACLS certification
• Active New Jersey Registered Nurse License

Scheduling Requirements:

• Day Shift, 40 hours per week
• This position requires every other weekend coverage
