SNF - MDS Coordinator at Orlando Senior Health Network – Orlando, Florida

JOB SUMMARY:

Has the responsibility of completing MDS assessments and develop care plans for residents. The MDS Coordinator will make certain the resident’s assessment care planning requirements are met and are consistent with State and Federal requirements and regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Completing a comprehensive resident assessment on admission and make required updates.
• Organize and schedule resident care plan meetings with the patient’s family and/or responsible party. Responsible for maintaining records and notes from the meeting.
• Responsible for timely completion of MDS by health care team members.
• Secure and transmit MDS records to state.
• Participate in QA meetings and facilitate audits when necessary.
• Provide in-services training on assessment practices of the facility.
• Assist residents when needed.
• Perform other duties as assigned.
