Position Summary:

The Clinical Care Coordinator is a Certified Diabetes Care and Education Specialist, or Registered Nurse credentialed professional who will work with patients to identify, evaluate, and recommend additional therapy interventions for current patients to optimize their quality of life. This position includes the role components of clinician, consultant, educator, and researcher.

Essential Functions and Job Responsibilities:

  • Evaluates nutrition status and determines nutrient needs in developing the nutrition care plan.
  • Adheres to the Academy of Nutrition & Dietetics Scope of Practice for Registered Dietitians.
  • Assigns risk levels and establishes ongoing follow up and monitoring according to department standards and protocols.
  • Serves as an expert clinical resource to staff, patients, and the community.
  • Supports staff and patients in developing advanced clinical skills through coaching, role modeling and educational strategies.
  • Provides expert clinical care when appropriate.
  • Contributes to the achievement of corporate objectives/goals by increasing referrals.
  • Participate in community education visits as requested by managers and leadership.
  • Assures a quality, cost-effective program to include on-going product evaluation and appropriate utilization of resources.
  • Participates in multidisciplinary case conferences to promote a teamwork approach among team members and peers when appropriate.
  • Identifies variances in standardized care and care processes to identify areas for improved performance related to patient care, documentation, and the organization.
  • Advocates for needed patient services or resources based on clearly communicating quantifiable information.
  • Coordinates care with others involved in patient care. Collaborates with all team members, and provides services needed to achieve patient and program goals.
  • Recommend clinical interventions working collaboratively with all health care team members to achieve desired results.
  • Provides educational services to staff, patients, and the community based on needs assessment.
  • Identifies and investigates clinical problems implementing appropriate strategies for resolution.
  • Utilizes outcome data to improve clinical practice.
  • Demonstrates the clinical interventions produce improved patient outcomes.
  • Research, implements, and reviews on-going new methods of charting to minimize documentation and charts.
  • Serves as a leader within the department, agency, and community.
  • Functions as a change agent in response to advances in practice, management, research, quality assurance and corporate objectives.
  • Collaborates with the management team to ensure a supportive environment for professional development.
  • Provides leadership in the development, coordination, and success of programs/projects.
  • Promotes customer service philosophy and responsible for department/teams’ impact on customer relations.
  • Participate in team and committee activities as requested.
  • Prioritizes own work to meet deadlines and project time frames.
  • Retain knowledge of and consistently adhere to procedures for the use of Personal Protective Equipment (PPE), infection control and hazardous materials handling.
  • Maintain patient confidentiality and function within the guidelines of HIPAA.
  • Complete assigned adherence training and other educational programs as required.
  • Maintain adherence with AdaptHealth’s Adherence Program.
  • Perform other related duties as assigned.
  • Able to make decisions for product use balancing clinical and nutritional needs with product line.
  • Electronically document patient care activities and all dietitian services provided. Documentation is accurate, complete, and follows company standards.
  • Provides education & training to patients and caregivers on use of equipment
  • Review orders and resupply needs
  • Maintain patient confidentiality and function within the guidelines of HIPAA
  • Perform other related duties as assigned.

Requirements:

Education and Experience Requirements:

  • Bachelor’s degree in nutrition related field
  • Valid and unrestricted RN or registered dietician license in state of residence
  • Two (2) years of clinical experience as an RN or CDCES
  • Valid and unrestricted driver’s license

Physical Demands and Work Environment:

  • Must be able to bend, stoop, stretch, stand, and sit for extended periods of time and ability lift to 10lbs as needed.
  • Ability to perform repetitive movements of the upper extremities’ motions of wrists, hands, and/or fingers due to extensive computer use
  • Work environment may be stressful at times, as overall office activities and work levels fluctuate
  • Subject to long periods of sitting and exposure to computer screen
  • Mental alertness to perform the essential functions of position.
  • Ability to work after non-business hours as needed.
  • May be exposed to angry or irate customers or patients
  • Must be able to drive and travel as needed
  • Physical and mental ability to provide clinical assessments
  • Excellent ability to effectively communicate both verbally and written with customers with the ability to demonstrate empathy, compassion, courtesy, and respect for privacy
  • Ability to work after non-business hours as needed.
