About This Position
INTRODUCTION:
Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.
ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following functions:
Oversees clinical care provided to patients to ensure that a high standard of care is achieved and maintained
Efficiently and effectively plans, organizes, directs, and helps implement the comprehensive nursing/clinical patient care program.
Facilitates cooperative effort amongst all disciplines as it relates to patient care
Maintains compliance with established CNCHC policies, procedures, and objectives, quality assessment and improvement programs, safety, environmental, and infection control standards
Helps create, direct, and implement an effective, ongoing program to measure, analyze, and improve the quality of nursing care, treatment, and services
Responsible for the efficient clinical workflow and ensuring medical support staff are contributing to productivity and utilization standards.
Collaborates with the Medical Directors, CMO, and Clinic Quality Committee to evaluate data and make institutional changes to improve Quality indicators and Quality Measures scores.
Collaborates with senior management to design and implement P4P programs and incentives for clinical support staff and providers.
Develops, implements, monitors, and adapts quality measures for CNCHC to ensure patient customer satisfaction, and that patients receive clinically appropriate treatment outcomes
Maintains the standards of nursing practices within CNCHC
Assists in assigning staff, provides comprehensive clinical training of clinical support staff, evaluation, and performance, and makes recommendations for personnel actions.
Assists in the implementation and use of standardized equipment and procedures for their use throughout the facility, when possible. Assists with the care and maintenance of equipment and medical supplies
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops
Attend meetings as required. Participates in medical, administrative, and educational committees as directed by the CMO and/or CEO
Reviews all incident reports for completeness and properly reports incidents to the relevant CNCHC departments in an effort to enhance teamwork to achieve the necessary change and adaptation to the CNCHC healthcare environment.
Demonstrates knowledge of and respects patients' service Providers, CNCHC confidentiality, and HIPAA procedures and protocols. Follow established HIPAA procedures when using and/or disclosing protected health information.
Maintains and protects patients’ rights under the HIPAA privacy standards
Knowledge of government regulations, CMS, COP, HRSA, TITLE 22
Excellent communication, leadership, and mentoring skills
Demonstrates competency to participate in interdisciplinary treatment teams
QUALIFICATION REQUIREMENTS:

EDUCATION:
Graduated from an accredited school of Nursing. College or university with a current registered nurse license by the state board of nursing
EXPERIENCE:
A minimum of 4 years of relevant clinical experience.
4 years of nursing management experience.
Licenses and Certification:
Current California RN license
Current CPR certificate
Current ACLS certificate

Excellent time management skills.
Strong analytical and problem-solving skills.
