Job Summary

As a Social Worker with Northfield Hospital + Clinics, you will provide services to address the needs of patients and family members through psychosocial assessment, discharge planning, emotional support, and providing resources.

Compensation

We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The annual wage range for this position is as follows, based on a 1.0 FTE: BSW $59,403.29 – $83,863.46; MSW $66,820.62 – $94,334.99. These figures reflect the base salary for the role.

Key Accountabilities

Provides psychosocial assessment for patients and their families to determine needs and appropriate services for all units of the facility including, medical /surgical, emergency room, birth center, and clinic. This position is located in Northfield and does not include travel to other clinic locations.
Provides social work services including supportive counseling, discharge planning, and community resource planning. Facilitates referrals for follow-up care.
Demonstrates the ability to work effectively with clients of all ages and with diverse cultural needs.
Develops and maintains a thorough knowledge of referral resources throughout the service area.
Works closely with members of the interdisciplinary health care team in development of plan of care.
Performs other related duties as assigned. Work is fast-paced and busy.

Qualifications

Licensed by the Minnesota State Board of Social Work, Master's level preferred.
2 years of social work experience in a health care environment preferred, including hospital or nursing home.

Schedule

48 hours per two-week pay period
8:00 a.m. – 4:30 p.m.
Three consistent days per week; flexible on specific days as long as the schedule remains consistent
