Clinical Liaison at Five Star Home Health Care – Fairfax, Virginia

As a Clinical Liaison, you are the face, voice and trusted partner representing our fully integrated in-home healthcare model. You will be the primary point of contact for cultivating and expanding relationships across hospitals, SNFs, physician practices and key healthcare partners throughout the Fairfax/Arlington/Alexandria region.

Location Requirement: Northern Virginia-based candidates only

Leveraging your existing referral network, you’ll identify and manage opportunities for our chronic disease management solutions at home.

Most representatives sell a single service line. You won’t.

• Own and grow strategic referral relationships across the market
• Drive consistent referral volume through proactive outreach and relationship farming
• Translate conversations into movement
