About This Position
Position Summary:The Client Care Liaison is a key member of the agency’s outreach and growth team, responsible for building strong relationships with referral sources, generating new client leads, and supporting the agency’s mission through community engagement and strategic marketing initiatives. This role combines sales, business development, and marketing efforts to expand awareness of services and increase client enrollment in the agency's homecare programs.Essential Duties and Responsibilities
Identify, cultivate, and manage relationships with referral sources including hospitals, discharge planners, physician offices, social service agencies, senior centers, and community-based organizations.Conduct outreach visits and presentations to promote the agency’s services and value proposition.Generate and follow up on referrals to convert leads into enrolled clients.Collaborate with intake and clinical teams to ensure smooth onboarding and transition of new clients.Implement business development strategies aligned with organizational goals and community needs.Track and analyze referral patterns and trends to optimize outreach and growth efforts.Represent the agency at health fairs, community events, and professional networking meetings.Support marketing initiatives including the development of flyers, brochures, digital campaigns, and event coordination.Maintain accurate records of outreach activities, client leads, and conversion outcomes in CRM or tracking systems.Provide feedback to leadership regarding market demands, competitive positioning, and partnership opportunities.
Qualifications:Bachelor’s degree in marketing, communications, healthcare administration, or related field preferred.Minimum of 2 years of experience in a sales, business development, or marketing role, preferably in healthcare, senior services, or nonprofit sectors.Proven ability to develop relationships and generate new business in a highly competitive environment.Strong communication, presentation, and interpersonal skills.Knowledge of NYC’s healthcare and human service networks strongly preferred.Passion for serving vulnerable populations and supporting the mission of a non-profit organization.Valid driver’s license or ability to travel within NYC boroughs regularly.Working Conditions:Hybrid-based role with up to four days per week in the community; alternatively in office, or home office with approval.Evenings and weekends required for community events or partner meetings or sales/census growth initiatives.Requires travel within all markets served and New York City and markets served; specifically being assigned to geographically.#HP
