Position: Case Manager – Bedford Hills & Taconic Correctional Facilities
Reports To: Program Director in each facility
Work Schedule: Monday to Friday, 9 am-5 pm (Weekends when needed)
Status: Full-time inside 2 NYS Women’s Correctional Facilities
Salary: $26.44 per hour
Position Summary:
Hour Children (HC) is an organization founded to help incarcerated and formerly incarcerated women and their children successfully rejoin the community, reunify with their families, and build healthy, independent, and secure lives. Duties and Responsibilities:
- Able to work with Incarcerated Individuals (II’s) inside a NYS Prison environment
- Work with Nursery Mothers to provide services for a successful reentry
- Ability to understand and comply with Grants and Funders contracts
- Assess care for the children of II’s by assessing treatment needs, evaluate, and develop plans and track progress
- Facilitate orientation when admitting new clients, review records and applications, work closely with counselors, housing specialists, and staff
- Maintain pristine and accurate data of client records and notes, events, and progress
- Enter all data into the agency's preferred platform/software on time
- Provide all collected data to the Impact Assessment Manager
- Provide reports by collecting and summarizing treatment and results data and trends, and compile statistics
- Complete all grant and subsidy applications and record data as directed in each grant or subsidy; be aware of time-sensitive reporting
- When creating a discharge plan, work with Offender Rehabilitation Counselors (ORC’s) to learn what programs parole is mandating and proceed to locate facilities where programs are available
- Provide resources to clients, families, and others, as needed
Qualifications:
- BA in Social Work or other relevant degree with 3 years’ experience or 5+ years’ Case Management experience, preferably in Human Services, with equivalent combination of training and expertise
- Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors
- Bilingual in English and Spanish is a plus
- Previous experience working with the adult community and reentry programming preferred
- Must have a valid-in-state driver’s license
- Experience with data entry; proficiency with computers
- Must know Microsoft Office, especially Word, Excel, and Outlook
- Must know how to navigate the Internet and electronic data
- Ability to work independently, be flexible and dependable, and demonstrate initiative
Specifications for Physical Requirements:
- Ability to walk around each facility and carry 20 pounds or less
Please Note: This role requires the successful completion and approval of a background check and application, to be conducted by the NYS Department of Corrections & Community Supervision in accordance with their protocols and procedures, in addition to an Hour Children application.
