Founded in 1978 and headquartered in New York City, SAGE is a national organization that offers supportive services and consumer resources for LGBTQ+ older adults and their caregivers, advocates for public policy changes that address the needs of LGBTQ+ older people, and provides cultural competency training for aging providers and LGBTQ+ organizations, largely through its National Resource Center on LGBTQ+ Aging and its growing social enterprise, SAGECare.
What we need:
The Director of Care Management provides overall leadership and accountability for SAGE’s Care Management programming across New York City. This role is responsible for contract management, program development and oversight, and the supervision of departmental staff. The Director directly supervises multiple team members and offers strategic leadership, guidance, and operational direction for all Care Management services. In collaboration with internal SAGE departments and external partners, the Director helps deliver on-site programs and activities that advance health, wellness, and a strong, vibrant community for LGBTQ+ older adults. This role will report to the Senior Director of SAGEServes Supportive Services.

This individual is an effective and experienced people manager who can mentor, develop, and hold multiple staff accountable while fostering an inclusive, collaborative, and high-performing team culture. They are highly organized and detail-oriented, with a strong understanding of regulatory and contractual compliance, and the ability to manage multiple priorities, deadlines, and stakeholders simultaneously.
The ideal candidate is also a strategic thinker who can contribute to the growth and evolution of Care Management programming while remaining engaged in day-to-day problem solving and operational decision-making. They demonstrate sound judgment, strong communication skills, and the ability to build and sustain relationships with LGBTQ+ older adults, colleagues, community partners, and public and private funders.
What you’ll do:
Program Leadership & Oversight
- Oversee Care Management services for LGBTQ+ older adults at all four SAGE Centers, including the Caregiving Program, Case Assistance, Information and Referral and other supportive programming
- Develop, implement, manage, support, and evaluate all Care Management Services programs, ensuring alignment with SAGE’s priorities and the achievement of contracted deliverables and outcomes
- Provide ongoing strategic direction to ensure high-quality, affirming, and responsive services
Staff Supervision & Workforce Development
- Provide weekly supervision to all direct reports, including social work interns
- Facilitate a weekly team meeting to review department updates, strategy, and case assignments
- Manage clinical supervision of staff in collaboration with the Senior Director of SAGEServes Supportive Services
- Direct the Care Management social work intern program
- Foster a collaborative, accountable, and supportive team culture
Fiscal Management & Grant Administration
- Manage and oversee departmental and program budgets funded by city, state, federal, and private sources
- Monitor spending to ensure alignment with funding requirements, program goals, and SAGE’s overall budget
- Work collaboratively with the Development and Finance teams to identify new funding opportunities and contribute to grant proposals
Compliance, Documentation & Quality Assurance
- Ensure compliance with program guidelines, funding requirements, and organizational policies
- Maintain complete, accurate, and timely documentation of all care management activities in paper and electronic systems
- Prepare for and participate in program audits with NYC Aging, DOHMH/GMHI, and other funders
External Relations & Advocacy
- Represent SAGE and the Care Management department to government agencies, private funders, and external committees or professional and networking groups
- Cultivate and maintain strong relationships with community partners, government agencies, and other stakeholders to advance SAGE’s mission and expand program impact
Organizational Leadership & Collaboration
- Represent the department at Director-level meetings and other organizational functions
- Foster cross-functional collaboration within SAGE, particularly with Finance, Development, and other departments
- Provide peer-level coverage of similarly scoped responsibilities during colleagues’ leaves or absences to ensure continuity of operations
- Other responsibilities and job duties as assigned
Required Qualifications:
- A minimum of 6 years of experience successfully managing multiple and complex funding sources and managing large teams
- LMSW required
- SIFI (Supervision in Field Instruction) certification required
- Strong organizational skills with a demonstrated track record of collaboration, leading and influencing progress of complex progress towards goals across an organization, and an understanding of relationship among Program, Finance, Human Resources and other stakeholders in ensuring successful outcomes
- Substantive knowledge or prior experience in aging or LGBTQ+ fields, including an understanding of issues facing marginalized older adults, particularly BIPOC and transgender elders
- Ability to plan and multi-task, and to produce under the pressure of deadlines and unexpected occurrences
- Demonstrated project management ability, involving complex and unpredictable variables
- Superior written and verbal communication skills
- Strong alignment with SAGE’s mission and values, with a passion for serving older LGBTQ+ adults and other marginalized communities.
Preferred Qualifications:
- Familiarity with NYC Aging, DOHMH, and/or similar regulations/systems/processes, as well as applicable government, nonprofit, and community social service programs serving low-income adults in New York City
- Experience with Salesforce and NYC Aging databases
- Strong understanding of performance measurement and metrics to evaluate program impact and effectiveness
- Experience in supervising and managing social work intern teams
- Familiarity with issues of cultural and linguistic competency with regards to ability, race, ethnicity, class, sexuality, gender identity and expression, and other characteristics that affect the lives of diverse older adults
- Fluency in Spanish (verbal and written)

Role Details:
- The status: This is a permanent, full-time, regular, exempt position
- The location and work schedule: ~35 hours per week, M-F 9:30AM-5:30PM, with occasional evenings and weekends required. This position requires working onsite at SAGE’s NYC headquarters at least two days per week.
- The salary: $105,000
