About This Position

Description:

The Housing Navigator is a critical role in the Community Supports program, which operates alongside the Enhanced Care Management (ECM) program. Community Supports provides targeted services to address social determinants of health, with housing recognized as a fundamental component of client stability and well-being.

The Housing Navigator is responsible for assisting clients in identifying, obtaining, and maintaining safe and stable housing. This includes coordinating with landlords and housing providers, facilitating access to community resources, and supporting clients in navigating housing-related barriers. While closely aligned with ECM care teams, the Housing Navigator functions within the Community Supports service line to ensure that housing needs are addressed as a specialized area of support. This role directly contributes to improved client outcomes, reduced health system utilization, and enhanced quality of life.

Starting Salary: $51,708.80

  • Associates or Bachelor’s degree in behavioral health, human services, public health, or related field strongly preferred.
  • Two years’ experience in housing navigation, social services, or related field with a focus on housing support.
  • Strong understanding of the housing sector, including affordable housing and fair-housing laws.
  • Excellent coordination and networking skills to work effectively with a variety of community partners and service providers.
  • Strong advocacy and interpersonal skills, with the ability to work compassionately with diverse populations.
  • Problem solving skills and the ability to think critically to overcome housing barriers.
  • This position requires local travel and the ability to navigate public transportation with clients.
  • Bi-lingual in English and Spanish is preferred.
  • Excellent interpersonal and communications skills and the ability to work effectively with a wide range of constituencies in the community and within the organization.
  • Be able to obtain Criminal and Child Abuse Clearance from the State of California/Department of Justice.
