The Community Connections Manager reports to the Executive Director and participates as a member of the interdisciplinary team creating a sense of belonging for every person who enters the home. The Community Connections Manager ensures that residents, families, visitors and team members feel known, valued, and supported through genuine human connection.

We’re looking for a compassionate, collaborative Personal Support Worker (PSW) who values meaningful relationships and understands that care is as much about heart as it is about skill.Located in beautiful Belle River, Ontario, this brand-new Residence, opening in 2026, will offer 96 single occupancy rooms with ensuite, and 64 shared rooms, each designed for comfort and dignity. Residents will enjoy 24-hour nursing care alongside a range of personalized supports that help them live well, feel valued, and stay connected to what matters most in their lives.Wage Rate: The starting annual salary is $50,000.Vacancy: This is not an existing vacancy and is a new position Responsibilities: Connection & Human Experience
Create a warm, welcoming and inclusive atmosphere for all residents, families, visitors, and team members. Support residents and families with information, guidance and connection, ensuring comfort and trust in their interactions with the Home.Know each resident and team member and purposefully weave connections between residents and team members, building relationships through daily conversations and authentic engagement;Maintain the front welcome hub and common areas, ensuring the space is inviting, organized, and reflective of home and community life. Collaborate with the Programs Manager on a calendar of social events for main entrance area, including art shows, exhibits and community engagement activities Storytelling & Culture Building Partner with the interdisciplinary team to share stories appropriately through internal and external communication channels, including displays, celebration spaces, bulletin boards, etc In the beginning, welcoming new team members and facilitating residents moving inResident and Community Support ServicesRespond to inquiries by phone or in person with professionalism, empathy and discretion, directing individuals to appropriate supports as neededMaintain visitor logs, deliveries, and correspondence accurately and confidentially. Provide administrative support, care conference booking, organizing resident transportation, mailing resident billings, liaising with community partners, office supply inventory, other administrative tasks as assignedSupporting the interdisciplinary team with the resident move-in process Supporting residents and families to navigate the Home’s services Support residents with access to their Resident Trust Accounts as needed Monitor front entrance to ensure resident safety and awareness of entry/exit activity Monitors the Home’s surveillance system Conduct monthly audits as assigned Collaboration & Communication Participate in daily or weekly team huddles to share resident and/or family feedbackShare home’s content of events and special moments with the Communication Team for website and social media posts.Manages the Optisign content and functionality Assist in the production and distribution of Home communiqués under the direction of the Executive Director Support facilitation of resident/family satisfaction surveys and team member engagement surveyParticipate in home-wide continuous quality improvement activities. Human Resources Management Support call-ins, calls on weekends Supporting the Human Resources functions as assigned Demonstrate good knowledge of Occupational Health & Safety Act, Labour Relations Act, Employment Standards Act and requirements under AODAParticipate in fire safety demonstrations and fire drills and is able to execute the Home’s emergency plan when requiredLeadership Provide leadership, guidance and supervision to Part-Time Community Connection CoordinatorsAssist with setting the vision, goals and objectives for the department in collaboration with team members and in line with the overall home's vision and strategic directions Has problem solving and conflict resolution skills and understands team concepts and team building. Other duties as assigned and/or indicated in Job Task Inventory Qualifications: The qualifications needed to join our family are as follows: Diploma in Social Services, Community Development, Communications, or Gerontology preferred. Experience in LTC, senior living, community support, or social services is an asset. Compassionate, approachable, and emotionally intelligent communicator. Modeling a person-centered living philosophy, embracing the Butterfly ModelStrong interpersonal and storytelling abilities; comfortable engaging with diverse individuals and families. Excellent organizational and administrative skills with attention to detail. Proficiency with Google suite, email, and basic digital photography or media tools. Excellent written and verbal communication skills Excellent organizational skillsMust provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements Two supervisory references required Provide proof of all required vaccines. The Employer strongly recommends to “stay up to date” with the recommended vaccines. Vaccination requirements are subject to changes as determined by the employer.
