ABP Best Homecare Agency, Inc., a well-established homecare provider based in Newton, MA, is seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to conduct Group Adult Foster Care (GAFC) and Adult Foster Care (AFC) home visits throughout the Greater Boston area.

? Responsibilities:

• Perform in-home nursing assessments for GAFC and AFC participants
• Monitor health conditions and update care plans as needed
• Educate clients and caregivers on medical needs and services
• Ensure accurate, timely documentation in accordance with MassHealth guidelines
• Collaborate closely with case managers and the clinical team

Requirements:

• Active RN or LPN license in Massachusetts
• Valid driver's license and reliable transportation
• Experience in home care or GAFC/AFC programs preferred
• Excellent communication and documentation skills
• Ability to manage time and work independently
