Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.Geriatric Nurse Practitioner / Physician Assistant EvergreenHealth, Kirkland, WA , Seattle East SideYou are invited to join our stellar Geriatric Medicine team at EvergreenHealth in Kirkland, WA.As a geriatric advance practitioner at EvergreenHealth, you will play a vital role in providing specialized medical care to our senior community. You will collaborate with a multidisciplinary team of healthcare professionals to ensure the well-being of residents while handling a range of services. The Geriatrician will diagnose and treat a broad range of medical conditions specific to the geriatric population, provide comprehensive care plans.Key Responsibilities:

• Conduct thorough medical assessments, diagnoses, and treatment plans for older adults
• Focus on management of polypharmacy
• Collaborate with the interdisciplinary team and specialists to develop and implement comprehensive care plans
• Provide guidance and education to staff, patients, and their families on geriatric care
• Participate in care conferences and interdisciplinary meetings
