CASE MANAGER RN- TOTAL LIFE HEALTHCARE at Saint Bernards Medical Center – JONESBORO, Arkansas

Degree/diploma in nursing and current State of Arkansas Registered Nurse license required. Bachelor's Degree in Nursing preferred.

A minimum of one year experience working with the frail or elderly population and case management experience. Clinical nursing experience preferred. Community and or geriatric nursing experience strongly preferred. Sound clinical skills and demonstrated good judgment. Working knowledge of physical, mental and social needs of the frail elderly population. Ability to problem-solve and plans effectively.
Effective oral and written communication skills. Basic computer skills.

Requires physical strength to perform the essential functions of the job.
Must be able to work required schedule. Occasional home visits may be required. Involves being inside and outside. Exposure to temperatures 32 degrees and less and 100 degrees or more, fumes, dust, and noise. Hearing within normal range. Frequent sitting, stooping, standing, walking, bending, pushing/pulling up to 250 pounds, carrying up to 35 pounds, and lifting up to 100 pounds. Must have reliable transportation and current drivers license valid in the state of Arkansas and automobile liability insurance.
